An investigation about prevalence of LV diastolic dysfunction (impaired relaxation) in 50 patients with type II diabetes mellitus, over 40 years old and without cardiac symptoms.

Introduction: Diabetes mellitus (DM) is the most common endocrine disease in the world today and the coronary heart disease is the major cause of human mortality in general population and in diabetic patients. So, the early diagnosis of cardiac disease in diabetic patients is important. In this survey, we studied 50 patients with DM type 2 with age over 40 and determined their left ventricular diastolic function (diastolic relaxation) via echocardiographic method. Materials and methods: This study was done in prospective and cross-sectional manner in Ardabil Bouali Hospital on outpatients and admitted diabetic patients and 50 type 2 diabetic patients with age over 40,that hadn’t clinical cardiac disease, were entered in the survey and echocardiographic study was done for them. Then the needed data were gathered from the patients and inserted in special forms. Finally the collected data were analyzed by statistical methods (like SPSS). Results: The mean age of 50 diabetic patients was 56 years, and 38 cases (76%) were female and 12 cases (24%) were male. In 96% (48 cases) of patients the left ventricular ejection fraction was > 50% and in 2% of patients (1 case) it was between 40-49% and in 2% (1 case) it was < 40%. In 24% of patients (12 cases), left ventricular diastolic function(relaxation) was normal (E >A), and in 76% (38 cases) there was diastolic dysfunction (impaired relaxation, E<A).One-third of patients with normal diastolic function were male and two-third were female, while in patients with impaired relaxation of left ventricle 21.1% were male and 78.9% were female. Conclusion: As mentioned above, it was clear in this study that many of type 2 diabetic patients with age over 40 and without obvious clinical cardiac disease, had left ventricular diastolic dysfunction (impaired relaxation)in echocardiographic study (76% versus 24%), while more intristingly, 96% of patients had normal left ventricular systolic function. So, it seems that left ventricular diastolic dysfunction(impaired relaxation) in type 2 diabetic patients can be generated even before clinical signs and symptoms and other cardiac findings.
