كارآيي آزمو نهاي فهرست واژگان و تصاوير ديداري در مقايسه با مصاحبه باليني در تشخيص اختلالات شخصيت

مقدمه: تشخيص اختلالات شخصيتي بر پايه طبقه بندي تشخيصي و آماري اختلالات رواني (DSM) محدوديت کاربردي دارد و به يک علم باليني نمي انجامد. اين پژوهش با هدف بررسي كارآيي آزمون واژگان و آزمون تصاوير جهت تشخيص و تشخيص افتراقي اختلالات شخصيت طراحي شد. روش کار: در اين مطالعه 328 نفر از افراد مشکوک به ابتلا به انواع اختلالات شخصيت به شيوه در دسترس از بين مراجعين به مراكز درماني و مشاوره اي و نيز زندانيان اردبيل از بهمن 1383 تا شهريور 1384 انتخاب شده و به آزمون محقق ساخته واژگان و تصاوير پاسخ دادند. از افراد فوق 45 نفر بهنجار بودند. داده ها با روش هاي آماري توصيفي و تحليل مميز مورد تحليل قرار گرفتند. يافته ها: آزمون واژگان در 96% موارد و آزمون تصاوير در 100% موارد به طور صحيح قادر به تفكيك انواع اختلالات شخصيت از يكديگر و از افراد بهنجار است. ضمنا آزمون واژگان تنها در مورد اختلالات شخصيت مرزي، پارانوئيد و اسكيزوتايپال در روش اجرايي انتخابي قادر به تشخيص مي باشد. اجراي آزمون واژگان به روش سنجش ميزان علاقمندي به صورت يک مقياس هفت رتبه اي نسبت به روش انتخاب صرف واژه ها کارآيي بهتري در تشخيص اختلالات شخصيت دارد. نتيجه گيري: آزمون واژگان و آزمون تصاوير جهت تشخيص انواع اختلالات شخصيت كارآيي مناسبي دارد اما در تاييد اين نتايج لازم است با آزمون هايي که در آن ها واژه ها و تصاوير به صورت دقيق تر انتخاب شده اند پژوهش هاي بيشتري انجام شود.

Efficacy of words list and illustrative pictures tests in diagnosis of personality disorders in comparison with clinical interview

Introduction: Diagnosis of personality disorders based on diagnostic and statistical manual of mental disorders (DSM) criteria has clinical limitation and is not considered as a clinical science. This survey is designed to investigate the efficacy of words list and illustrative pictures tests in personality disorders diagnosis and for propounding differential diagnosis of personality disorders. Materials and Methods: In this study, which was conducted during February 2004 to September 2005, 328 persons who seemed to be afflicted with different personality disorders were selected with random sampling from among those who had referred to therapeutic and consultation centers in Ardabil as well as Ardabil Jail prisoners. They responded to researcheredited words and pictures tests. Out of the mentioned participants, 45 were normal ones. Obtained data were analyzed employing descriptive statistics and discriminative analysis. Results: The findings showed that both words list and illustrative pictures tests can distinguish different personality disorders from normal persons in 96% and 100% of cases, respectively. Words list test can only diagnose borderline, paranoid, and schizotypal personality disorders in the selective executive method. Also, words list test, as a 7 degrees scale for diagnosis of personality disorders, is more useful than selection only words list. Conclusion: Words list and illustrative pictures tests are useful for diagnosis of personality disorders; it is also suggested that further studies be done with the mentioned tests employing more precision in choosing words and pictures.
