ارزیابی کارکرد بیمارستان‌های دانشگاه علوم پزشکی گیلان با رویکرد DEA و در حضور عوامل ناپسند، پیش و پس از اجرای طرح تحول نظام سلامت

Persian Abstract

مقدمه: تغییر در نظام سلامت‌ همواره از دغدغه‌های بنیادی دولت‌ها به شمار می‌رود و می‌تواند آثار مثبت و منفی بسیاری ‌‌بدنبال داشته باشد. طرح تحول نظام سلامت از‌‌ برنامه‌هایی است که با هدف بهبود ابعاد درمانی و اقتصادی خانواده‌ها‌ در نخستینگی برنامه‌های وزارت بهداشت درمان و آموزش پزشکی دولت یازدهم قرار گرفت و پیش بینی می‌شود افزون بر تحول نظام آموزشی، بهداشتی، پژوهشی و فرهنگی دانشگاه‌های علوم پزشکی، سبب بهبود عملکرد آنها و بیمارستان‌های زیر نظر آنها شود. هدف: سنجش عملکرد 22 بیمارستان دولتی دانشگاه علوم پزشکی گیلان پیش (فروردین 92 الی فروردین 93) و پس (اردیبهشت 93 الی اردیبهشت 94) از اجرای طرح تحول نظام سلامت، با استفاده از رویکرد واکاوی پوششی داده‌ها ‌‌با توجه به تولید عوامل نامطلوب. مواد و روش‌ها: این پژوهش، ‌‌مطالعه‌ای توصیفی_ تحلیلی و‌‌ مقطعی و جامعه آماری‌، دربرگیرنده 22 بیمارستان وابسته به دانشگاه علوم پزشکی گیلان بود و نمونه‌گیری از آن به‌ صورت سرشماری و گردآوری داده‌ها با گرد‌آوری گواه‌مندی‌ها انجام شد. در پایان، داده‌ها‌، با برنامه‌نویسی الگوی‌ دسترسی پذیری ناتوان تحلیل پوششی داده‌ها در نرم‌افزار GAMS واکاوی شد. نتایج: یافته‌ها نشان داد که در سال 1392 و پیش از اجرای طرح تحول نظام سلامت، 14 بیمارستان (63%) کارا بودند که این تعداد پس از اجرای طرح تحول نظام سلامت و در سال 1393، به 16 بیمارستان (73%) افزایش یافت. متوسط کارایی فنی نیز به‌ترتیب 954/0 و 958/0 محاسبه شد. نتیجه‌گیری: ‌اگرچه دیدن آثار تغییر در نظام‌های اجتماعی نیازمند گذشت زمان کافی است، با این حال، گذشت یکسال از پیاده‌سازی طرح تحول نظام سلامت نشان می‌دهد که متوسط عملکرد بیمارستان‌های دولتی استان گیلان بهبود یافته است

Performance Evaluation of Hospitals Affiliated With Guilan University of Medical Sciences, Based on a DEA Approach and in the Presence of Undesirable Factors Before and After Health Sector Development Plan

English Abstract

Introduction: Change in the health system is always considered as the main concern of governments which in turn may have many positive and negative effects. The health sector development plan is one of the programs aiming at improving the therapeutic and economic dimensions of families, given priority by the 11th government of Iran, and is expected to improve performance of the medical sciences universities and their affiliated hospitals, in addition to develop their educational, health, research and cultural systems. Objective: The aim of this study is to evaluate technical efficiency of 22 public hospitals of Guilan university of medical science before (April 2013 to March 2014) and after (April 2014 to March 2015) implementation of health sector development plan, using data envelopment analysis approach and in accordance with the production of undesirable factors. Materials and Methods The current research is a descriptive-analytical type and also a cross sectional one. The research population includes 22 public hospitals of Guilan University of Medical Sciences, with enumeration chosen as the sampling method and the data were collected from documents. Finally, the collected data were analyzed based on the coding the DEA weak disposability model in GAMS software. Results: The findings indicate that before implementation of health sector development plan (2013- 2014), 14 hospitals (63%) were efficient, while after its implementation (2014-2015), number of efficient hospitals increased to 16 units (73%). The average of technical efficiency also computed as 0.954 and 0.958, respectively. Conclusion: Although, observing the effects of changes in social systems often requires time lapse, however, one year after the implementation of the health system development plan, the average of performance in public hospitals of Guilan province was improved.
