Comparisons of the eosinophil cationic protein (ECP) levels and eosinophil counts in sputum of mustard lung disease (MLD) and normal subjects

English Abstract

Introduction: Sulfur mustard(SM) as a chemical weapon was used during Iran-Iraq war. The aim of this study was to evaluate the sputum level of eosinophilic cationic protein (ECP) and eosinophilic count and comparisons with healthy subjects to determine the presence of eosinophilic inflammation in MLD subjects. Methods: Forty MLD subjects with history of mustard exposure were enrolled in study. Also 15 healthy men were considered as control group.After salin nebulization, the sputum from subjects and healthy controls were collected and homogenized with four time volumes of 0.1% dithiothreitol. The concentration of ECP and eosinophilic count in the induced sputum were measured. Results: This is the first study using induced sputum to measure air way inflammatory marker in MLD. Sputum ECP in subjects and controls were 46.1±19.5 and 23.6±19.5 respectively (p<0.01). Sputum eosinophilic count of subjects and controls were 11.7%±11.1 and 4.3%±5.9 respectively (p=0.04). Statistically significant differences were found between the ECP and eosinophilic counts of the sputum in subjects and controls. There were significant differences between sputum severity of eosinophilic inflammation (1%, 2% or more than 3%) and ECP concentration of sputum (p<0.01). The comparison of Sputum ECP in bronchodilator responder subjects with non-responders showed statistically significant difference (p=0.01). Conclusion: We conclude that there is significant difference between ECP concentration and eosinophilic count in sputum as inflammatory markers in MLD compared to controls. Probably declining in these inflammatory markers will be useful in MLD patients.
