تاثير رژيم غذايي غنی از مكمل اسيد لينولئيك مزدوج ( CLA) بر سطح اینترلوکین 6 (IL-6) در بيماران مبتلا به COPD

سابقه و هدف: بیماری مزمن انسدادی ریه (COPD) با التهاب سیستمیک و التهاب تسریع‌شده‌ی ریه‌ها مشخص می‌گردد. برخی مطالعات حاکی از تأثیر مثبت اسید لینولئيک مزدوج (CLA) بر رفع التهاب می‌باشد. هدف اصلی مطالعه‌ی حاضر تأثیر استفاده از مکمل‌های CLA بر سطح سرمی اینترلوکین 6 در بیماران مبتلا به COPD می‌باشد. مواد و روش‌ها: 82 بیمار با COPD اثبات شده وارد مطالعه‌ی کارآزمایی بالینی دوسوکور حاضر شدند. بیماران، به صورت تصادفی در دو گروه دارونما (تعداد=42) و مداخله (تعداد=40 نفر؛ دریافت 3/2 گرم CLA به صورت روزانه) وارد شدند. حجم بازدمی خارج شده فشاری در ثانیه اول (FEV1)، امتیاز آزمون ارزیابی COPD (CAT score) و سطوح سرمی اینترلوکین 6 در دو نوبت قبل از شروع مداخله و 6 هفته بعد از شروع مداخله بر اساس معیار‌های جهانی بیماری‌ انسدادی ریه (GOLD) برآورد شدند. یافته ها: بعد از تجویز مکمل‌های CLA سطوح سرمی اینترلوکین 6 و امتیاز CAT به صورت معنی‌داری کاهش یافت ( با سطوح معنی‌داری 0/05>p و 0/001>p). بعلاوه، FEV1 در گروه مداخله به صورت معنی‌داری افزایش داشت (0/001>p). بر اساس معیار‌های GOLD، کاهش سطوح سرمی اینترلوکین 6 در مراحل III-IV گروه مکمل به لحاظ آماری معنی‌دار بود (0/01>p). نتیجه‌گیری: تجویز مکمل‌های CLA می‌تواند منجر به بهبود مارکر‌های التهابی و شاخص‌های ارزیابی سلامت در بیماران COPD شود. نتایج حاصل پیشنهاد می‌کنند که می‌توان از رژیم درمانی واجد مکمل‌های CLA در مدیریت بیماران مبتلا به COPD بهره برد.

Effect of Conjugated linoleic acid supplemented diet on levels of interlukin-6 in COPD patients

English Abstract

Background and Objective: Chronic obstructive pulmonary disease (COPD) is characterized with systemic inflammation and accelerated inflammaging of the lungs. Some studies have shown that conjugated linoleic acid (CLA) has anti-inflammatory effects. The aim of the present study was to evaluate the effect of CLA supplementation on the serum levels of interleukin (IL-6) in patients with COPD. Methods: 82 patients with stable COPD were selected to be studied in a double blind clinical trial. Subjects were randomly assigned to two groups: placebo (n=42) and supplementation (n=40, 3.2 g CLA daily). Forced expiratory volume in one second (FEV1%), CAT score and the serum levels of IL-6 were measured at the baseline and also six weeks after the intervention. In addition, the study parameters in the two groups were compared based on the Global Initiative for Chronic Obstructive Lung Disease (GOLD) criteria. Results: After supplementation with CLA, the serum levels of IL-6 and CAT score significantly decreased (p<0.05 and p<0.001 respectively). In addition, the FEV1 (p<0.001) significantly increased in the supplementation group. Based on GOLD criteria, the decrease in IL-6 serum levels were found to be statistically significant in stages III-IV in the supplementation group (P<0.01) Conclusions: Supplementation with CLA can modify the inflammatory markers and improve the health status of COPD patients. The results suggest that CLA supplementation in COPD patients can be useful in the management of the disease
