چالش های محلی برای اجرای برنامه های آموزش سلامت در خانه ها و مراکز بهداشتی روستایی: یک مطالعه کیفی

زمینه: آموزش بهداشت یکی از اصلی ترین مراقبتهای اولیه بهداشتی (PHC) است. با این حال، شواهد محدودی بر مشکلات اجرای برنامه های آموزش بهداشت وجود دارد. این مطالعه به بررسی موانع اجرای برنامه های آموزش بهداشت در جوامع روستایی ایران پرداخته است. اهداف. یک مطالعه کیفی با روش تحلیل محتوای متعارف انجام شد. با استفاده از نمونه گیری هدفمند، 34 نفر از افراد و تامین کنندگان مراقبت های بهداشتی مشغول به کار در روستا در این مطالعه مشارکت کردند. داده ها از طريق مصاحبه های نيمه ساختاري جمع آوري شد. تجزیه و تحلیل داده ها تا زمان اشباع داده ها ادامه پیدا کند، زمانی که هیچ تم یا ایده جدید ظاهر نشد، مصاحبه متوقف گردید. مواد و روش ها: یک مطالعه کیفی با روش تحلیل محتوای متعارف انجام شد. با استفاده از نمونه گیری هدفمند، 34 نفر از مردم روستایی و تامین کنندگان مراقبت های بهداشتی مشغول به خدمت در خانه های بهداشت و مراکز بهداشتی روستایی در این مطالعه وارد شدند. داده ها به صورت جداگانه، تا زمانی که اشباع داده ها ادامه پیدا کند تجزیه و تحلیل شد. نتایج: چهار تم، از جمله "فرآیندهای آموزش و یادگیری ناکارآمد"، "فقدان انگیزه مربیان بهداشت"، " شکاف ارتباطاتی" و" کمبود منابع و تسهیلات برای آموزش و یادگیری "به عنوان موانع اجرای آموزش برنامه های آموزش سلامت در جوامع روستایی استخراج گردید. نتیجه گیری: چندین مشکل اجرایی و ارتباطی به عنوان موانع اجرای آموزش بهداشت در سطح محلی شناسایی شد. درک بهتر مدیران درباره طیف وسیعی از موانع آموزش بهداشت در مناطق روستایی ممکن است مفید باشد. همچنین، برای کارکنان مراکز بهداشتی روستایی و سایر ذینفعان، طراحی و یا بازنگری برنامه های آموزش بهداشت در جوامع روستایی پیشنهاد می گردد.

Local-level challenges to implementing health education programs in rural settings: a qualitative study

Background. Health education is one of the main cores of primary health care (PHC). However, there is limited evidence on the difficulties of implementing health education programs. This study explored the barriers of implementing health education programs in Iranian rural communities. Objectives. A qualitative study with conventional content analysis approach was conducted. Applying purposeful sampling, 34 rural folks and health care providers were employed to participate in the study. Data were collected through individually, semi-structured interviews. Four themes, including “Ineffective teaching and learning processes”, “Lack of health educators’ motivation”, “Communication gaps”, and “Lack of resources and facilities for teaching and learning” emerged as the barriers of implementing health education programs in rural communities. Conclusions. Several executive and communicational problems were identified as the local-level obstacles of implementing health education programs in rural areas. Better understanding on the extensive range of health education barriers in rural areas may be helpful for rural health workers and stakeholders in designing and/or revisiting health education programs in rural communities.
