مقایسه تأثیر سه نوع سمان بر تطابق لبه‌ای کوپینگ های تمام سرامیک zirconia در دو نوع طرح خط خاتمه تراش

Persian Abstract

مقدمه: تطابق لبه ای یک ویژگی مهمی در موفقیت طولانی مدت کلینیکی روکش می‌باشد که تحت تأثیر طرح خط خاتمه تراش و سمان مورد استفاده برای سمان کردن قرار می‌گیرد. بنابراین این مطالعه با هدف ارزیابی تطابق لبه‌ای کوپینگ های سرامیکی زیرکونیا در دو طرح خط خاتمه تراش که توسط سه نوع سمان مختلف سمان شده است، انجام شد. مواد و روشها: یک طرح تراش Deep chamfer روی یک دندان پرمولر دوم ماگزیلا طراحی شد، سپس به طرح تراش Shoulder تغییر شکل داده شد. دو دای با استفاده از سیستم CAD/CAM تراش داده شدند. 21 عدد قالب از هر دای اصلی گرفته شد و گچ آن‌ها ریخته شد. سپس 21 عدد کوپینگ زیرکونیا توسط سیستم CAD/CAM از هر دای اصلی ساخته شد. سپس نمونه ها توسط سمان گلاس آینومر، زینک فسفات و سمان رزینی سمان شدند. در نهایت گپ مارجینال 42 عدد کوپینگ زیرکونیا با استفاده از یک استریو میکروسکوپ در چهار نقطه ثابت (باکال، پاکال، مزیال و دیستال) مورد ارزیابی قرار گرفت. داده‌ها به وسیله آزمون Anova با سطح معنی‌داری از کمتر از 0/05 آنالیز شد. نتایج: کمترین مقدار میانگین کلی گپ مارجینال به ترتیب با سمان گلاس آینومر در طرح تراش (Deep chamfer (17/23±μm 5/79 با سمان رزینی در طرح تراش Shoulder 24/09±μm35/22؛ با سمان رزینی در طرح تراش (Deep chamfer (35/22±μm24/09؛ با سمان گلاس آینومر در طرح تراش (Shoulder (48/37±μm10/40؛ سپس با سمان زینک فسفات در طرح تراش (Shoulder (52/93± μm15 /44؛ و در نهایت با سمان زینک فسفات در طرح تراش Deep chamfer 58/89±μm14/90 برآورد گردید. نتیجه‌گیری: میانگین کلی تمام مقادیر گپ مارجینال در محدوده قابل قبول کلینیکی بود. در طرح تراش Deep Chamfer تفاوت معنی‌داری در تطابق لبه‌ای بین سمان گلاس آینومر و زینک فسفات وجود داشت اما تفاوت معنی‌داری بین سمان های مختلف دیگر مشاهده نشد. تفاوت معنی‌داری در تطابق لبه‌ای با طرح تراش Shoulder بین سمان های مختلف وجود نداشت.

Comparison of the effect of three types of cements on the marginal adaption of all ceramic zirconia coping crowns in two different types of finishing line designs

English Abstract

Introduction: Marginal integrity is an important characteristic in clinical long-term restoration succes,which is influenced by the finishing line design and the type of cement applied for cementation. So this study aimed to evaluate the marginal adaptation of zirconia coping ceramic crowns with two different finishing line designs cemented by three different types of cements. Materials and Methods: A Deep chamfer finishing line was designed on a maxillary second premolar,then trasformed into Shoulder. Two dies were milled using CAD/CAM system.21 impressions were made from each master die and poured.Then 21 zirconia copings were manufactured by CAD/CAM system from each master die. Then cemented by Glass ionomer,Zinc phosphate and Resin cement. Finally the marginal fit of 42 zirconia copings was evaluated using a Stereomicroscope in four points on each aspect (Buccal ,Palatal ,Mesial & Distal).The data were statistically analysed using ANOVA test.(P<0.05) Results: The lowest overall mean value of marginal Gap was recorded with Glass Ionomer cement in Deep Chamfer finishing line design(17.23 µm±5.79),with Resin cement in Shoulder finishing line design(35.22 µm±24.09),with Resin cement in Deep Chamfer finishing line design (35.23 µm±21.78) ,with Glass Ionomer cement in Shoulder finishing line design(48.37 µm±10.40) then with Zinc Phosphate cement in Shoulder finishing line design(52.93 µm±15.44) and finally with Zinc Phosphate cement in Deep Chamfer finishing line design(58.89 µm±14.90). Conclusion: The overall mean values of marginal adaptation were within the clinical acceptable range. In Deep chamfer finishing line design There was significant difference in marginal integrity between Glass ionomer and Zinc phosphate cement but no significant difference between other different cements.There was no significant difference in marginal integrity with Shoulder finishing line design between different cements.
