تاثیر مراقبت مبتنی بر پروتکل بارو بر سلامت دهان بیماران تحت درمان با تهویه مصنوعی بستری در بخش مراقبت های ویژه

Persian Abstract

زمینه و هدف: یکی از مراقبت مهم بیماران در بخش مراقبت های ویژه، مراقبت از دهان بیمار می باشد. در واقع، مراقبت از دهان از عوارض جدی دیگر مانند پنومونی ناشی از تهویه مصنوعی جلوگیری می کند. این مطالعه با هدف تعیین تاثیر مراقبت مبتنی بر پروتکل بارو در بیماران تحت درمان با تهویه مصنوعی بر روی بیماران بستری در بخش مراقبت های ویژه بیمارستان فاطمی اردبیل انجام شد. روش: مطالعه حاضر یک کارآزمایی بالینی دوسوکور بود که در سال 98-97 روي 90 بیمار بستري در بخش های مراقبت ویژه انجام گرفت. بیماران به روش تصادفی سازی بلوک بندی در دو گروه روتین و مداخله قرار گرفتند. براي بیماران گروه مداخله، یک برنامه ترکیبی مراقبت از دهان براساس پروتکل تعدیل شده بارو به مدت 5 روز انجام شد. وضعیت سلامت دهان طی پنج روز مطالعه در هر دو گروه ارزیابی و مورد مقایسه قرار گرفت. داده ها در نهایت بعد از جمع آوری با استفاده از نرم افزار SPSS نسخه 26 تحلیل شد. برای تحلیل داده ها از آمار توصیفی و استنباطی( آزمون کای اسکوئر، تی مستقل، آزمون آماری تحلیل واریانس در اندازه گیری های تکراری) استفاده شد. نتایج: نتایج نشان داد که تفاوت آماري معناداري در نمرات مقیاس ارزیابی سلامت دهان روزهاي اول و دوم در دوگروه مداخله و روتین وجود نداشت ولی در روزهاي سوم، چهارم و پنجم وضعیت بهداشت دهان گروه مداخله به طور معناداري، بهتر از گروه کنترل بود (0/001>p). نتیجه گیری: بر اساس نتایج پژوهش حاضر می توان نتیجه گیری کرد که مراقبت از دهان مبتنی بر پروتکل تعدیل شده بارو منجر به بهبود وضعیت سلامت دهان بیماران بستري در بخشهاي ویژه می شود. بنابراین، توصیه می شود این پروتکل در بخش های مراقبت ویژه مورد استفاده قرار بگیرد.

The effect of Barrow Oral Care Protocol on Oral Health in Patients Receiving Mechanical Ventilation Admitted in the Intensive Care Unit

English Abstract

Background and Aim: One of the most important care of patients in intensive care unit is patient's oral care. In fact, oral care prevents other serious complications such as pneumonia caused by artificial ventilation. The aim of this study was to determine the effect of Barrow Protocol based care on patients admitted to the intensive care unit of Ardebil Fatemi Hospital. Method: This study was a double-blind clinical trial design performed on 90 patients admitted to ICU in 2018-19. Patients were randomly divided into intervention and control groups. For the intervention group patients, a combined oral care program was performed based on the modified Barrow protocol for 5 days. Oral health status was evaluated and compared in both groups during the five days of study. The data were finally analyzed using SPSS software version 26. Descriptive and inferential statistics (Chi-square test, independent t-test, ANOVA for repeated measures) were used for data analysis. Results: There was no statistically significant difference in oral health of first and second days between the routine and intervention groups but the difference started from the third day and continued until the last day of study p<0.001). Conclusion: Based on the results of this study, it can be concluded that Barrow oral care Protocol improves oral health status of patients receiving mechanical ventilation in intensive care units. Therefore, it is recommended that this protocol be used in intensive care units.
