بررسي كارائي روش بروسلا كپت با تست هاي سرولوژيك رايج در تشخيص بروسلوز انساني

Persian Abstract

مقدمه: بیماری بروسلوز در ایران آندمیک است. از آنجائیکه این بیماری بسیاری از اعضای بدن را درگیر می نماید و علائم بیماری غیر اختصاصی می¬باشد، تشخیص بیماری براساس یافته¬های بالینی دشوار بوده و امکان اشتباه در تشخیص وجود دارد. از آزمایشات سرولوزیک بسیاری برای تشخیص بروسلوز انسانی استفاده شده است. در این مطالعه، ارزش تشخیصی تستهای رایت، الیزا (IgG+IgM)، ME2، کمبس رایت و بروسلاکپت در افراد مشکوک به بروسلوز تعیین شده است. متد و بیماران: افراد مورد مطالعه را 10 بیمار بروسلوزی با کشت خون مثبت و 47 فرد مشکوک به بروسلوز با کشت خون منفی تشکیل می دادند. نمونه های سرم از این افراد جمع آوری گردیده و با تست بروسلاکپت و سایر تستهای سرولوژیک متداول مورد آزمایش قرار گرفتند. نتایج: در بیمارانی با کشت خون مثبت، تست رایت در 4 بیمار از 10 بیمار و تستهای بروسلاکپت و کمبس رایت در 7 نفر و تستهای الیزای (IgG+IgM) در تمام بیماران مثبت بودند. در 47 بیمار مشکوک با کشت خون منفی، الیزا (IgG+IgM)، رایت، بروسلاکپت به ترتیب در 28، 18 و 21 بیمار مثبت بودند. بحث: براساس نتایج این مطالعه، هیچ یک از تستهای سرولوژیک به تنهایی در تمام افراد مشکوک و افراد دارای بروسلوزی تأیید شده، مثبت نبودند. به این دلیل، ما قویاً پیشنهاد می کنیم که برای تشخیص بروسلوز از مجموعه ای از تستهای سرولوژیک استفاده شود. حساسیت آزمایش الیزا (IgG+IgM)، از سایر تستهایی که بطور متداول برای تشخیص بروسلوز به کار می روند بیشتر بود.

The evaluation of efficacy of brucellacapt method and routine serologic methods for diagnosis of human brucellosis

English Abstract

Introduction: Brucellosis is endemic in Iran. Since it affects many organs and the symptoms are non-pecific, the diagnosis by clinical findings is difficult and may be easily missed. Many serological tests have been used for the diagnosis of human brucellosis. This study determined the diagnostic value of the Brucella standard tube agglutination test (SAT), ELISA (Brucella specific IgG and IgM) tests, 2ME Test, combs test and Brucellacapt test in suspected patients with brucellosis. Patients & Methods: Ten patients with brucellosis who had positive blood cultures for Brucella species, and 47 suspected patients were included in the study. Serum samples collected from these patients were tested by brucellacapt and conventional serological tests. Results: In the patients with blood culture positive, 4 of 10 samples gave positive results with SAT, 7 with both coombs and brucellacapt tests, and all patients were positive with ELISA (IgG + IgM). Out of 47 with blood culture negative, ELISA (IgG + IgM). SAT, Brucellacapt were positive in 28, 18, 21 patients respectively. Conclusion: According to this study, none of serological tests alone was positive in all suspected and confirmed patients with brucellosis. For this reason, we strongly suggested the use of combination of tests for diagnosis of brucellosis. The sensitivity of ELISA (IgG + IgM) was higher than conventional tests.
