بررسی رابطه استانداردهای اعتباربخشی بیمارستانها برشاخص های عملکردی اورژانس آنها

زمینه: اعتباربخشی بیمارستانی به عنوان یکی از روش‌های ارزشیابی رسمی و سیستمی خارجی بیمارستان‌ها نقش بسزایی در بهبود کیفیت، ایمنی و اثربخشی خدمات بیمارستان‌ها دارد. هدف: هدف از انجام این مطالعه بررسی تأثیر رعایت استانداردهای اعتباربخشی بر شاخص‌های عملکردی اورژانس بیمارستان‌های استان اردبیل بود. مواد و روش ها: این پژوهش توصیفی- تحلیلی به صورت مقطعی در بخش‌ اورژانس ۱۲ بیمارستان استان اردبیل انجام شد. اطلاعات مرتبط با امتیاز اعتباربخشی و پنج شاخص عملکردی میانگین مدت زمان تریاژ، درصد احیای قلبی ریوی ناموفق، درصد بیماران تعیین تکلیف شده ظرف مدت ۶ساعت، درصد بیماران بستری موقت خارج شده از اورژانس ظرف مدت ۱۲ ساعت و درصد ترک بیماران از اورژانس با مسئولیت شخصی در سال‌ ۱۳۹۹ جمع‌آوری شد. داده‌ها با استفاده از نرم‌افزار آماری SPSS نسخه ۲۲ و آمار توصیفی و استنباطی تحلیل شدند. نتایج: نتایج بررسی ۱۲ بیمارستان در استان اردبیل نشان داد که تعیین تکلیف زیر ۶ ساعت در اورژانس (91/77درصد)، درصد بیماران خارج‌شده از اورژانس در مدت ۱۲ ساعت (95/18 درصد)، درصد CPR ناموفق (78/14 درصد)، ترک اورژانس با مسئولیت شخصی (10/05)، میانگین زمان تریاژ در سطح ۱ (0/75)، میانگین زمان تریاژ در سطح ۲ (1/99)، میانگین زمان تریاژ در سطح ۳ (4/12)، میانگین زمان تریاژ در سطح ۴ (6/92) و میانگین زمان تریاژ در سطح ۵ (8/45) بدست آمد، همچنین رابطه ی معناداری بین استانداردهای اعتباربخشی بیمارستان ها بر شاخص های عملکردی اورژانس آنها وجود نداشت (0/05 <P). نتیجه گیری: رعایت استانداردهای اعتباربخشی منجر به بهبود شاخص‌های عملکردی بخش اورژانس بیمارستان‌های استان اردبیل نشد؛ بنابراین ارتقای سیستم اعتباربخشی بخش اورژانس بیمارستان‌ها شامل استانداردها، روش و ارزیابان اعتباربخشی و تعهد مدیران بیمارستان‌ها برای استفاده از سیستم‌های مدیریت کیفیت به منظور بهبود شاخص‌های عملکردی ضروری است.

Study The Relationship Between Aooreditation Standards Of Hospitals On Their Emergenoy Performance Indicators

English Abstract

Background: Hospital accreditation as one the means of formal and systematic external evaluation, has a major impact on improving the quality, immunity, and efficacy of hospitals’ services. Aim: The purpose of this study was to evaluate the effectiveness of complying the accreditation standards on performance indicators of the emergency departments of hospitals in Ardabil province. Materials and Methods: This descriptive_analytical study was conducted cross_sectionally in emergency departments of 12 hospitals in Ardabil Province. Data concerning the accreditation scores and five performance indicators including the average time of triage, the percentage of unsuccessful CPRs, the percentage of patients with determined medical plan within 6 hours, the percentage of temporarily admitted patients who were discharged from the emergency department within 12 hours, and the percentage of the patients who left the emergency department with their own consent, was collected in 2020. Data was analyzed by the 22nd version of the analytical software SPSS and descriptive and inferential statistics. Results: The results of the evaluation of 12 hospitals in Ardabil Province proved as follows: the percentage of patients with determined medical plan within 6 hours was 91.77% , the percentage of patients discharged from the hospital within 12 hours was 95.18%, the percentage of unsuccessful CPRs was 78.14 %, the average time of triage was 1:99 minutes in level 2, 4:12 minutes in level 3, 6:92 minutes in level 4, and 8:45 minutes in level 5. There was no significant relationship between the hospitals’ accreditation standards on performance indicators of their emergency departments, as well. Conclusion: Complying the accreditation standards did not lead to improvement of the performance indicators of emergency departments of the hospitals in Ardabil Province; therefore, upgrading the accreditation system of emergency departments of hospitals including standards, methods, people in charge with the accreditation, and the commitment of hospital managers to apply the quality management systems for improving the performance indicators is crucial
