بررسی روند ارائه خدمت جراحی طی پاندمی کرونا و قبل از آن در بیمارستان های دکتر فاطمی و امام خمینی (ره) اردبیل

زمینه: کووید 19 در سطح جهانی نظام سلامت را تحت تنش قرار داده و به نظر می رسد بسیاری از خدمات از جمله خدمات جراحی از این بیماری جهان گیر متاثر شده است. ما در این مطالعه به بررسی روند ارائه خدمت جراحی طی پاندمی کرونا و قبل از آن (سال 1399 نسبت به سال 1398) در بیمارستان های دکتر فاطمی و امام خمینی (ره) اردبیل پرداخته ایم. هدف: تعیین روند ارائه خدمت جراحی طی پاندمی کرونا و قبل از آن در بیمارستان های دکتر فاطمی و امام خمینی (ره) اردبیل مواد و روش کار: در این مطالعه مقطعی تحلیلی، داده های 1028 نفر از بیمارانی که در بیمارستان های امام خمینی (ره) و فاطمی اردبیل طی سال های 98 و 99 (قبل و طی پاندمی کرونا) تحت عمل جراحی قرار گرفته بودند وارد مطالعه شدند و متغیرهای مورد نیاز شامل نوع جراحی، سن، جنس، وزن، فشار خون، فاصله زمانی تشخیص اولیه تا جراحی، سرویس جراحی، و مدت بستری برای همه تعیین و در چک لیست های از پیش طراحی شده ثبت شد. یافته‌ها: تعداد جراحی ها در سال 1399 نسبت به سال 1398 در بیمارستان امام خمینی 59/8 درصد و در بیمارستان فاطمی 11/4 درصد کاهش یافته بود. فراوانی اعمال جراحی در سال 1398 و 1399 به تفکیک سرویس جراحی در هر دو بیمارستان امام خمینی (P=0/011) و فاطمی (P=0/007) و همچنین به تفکیک نوع جراحی در هر دو بیمارستان امام خمینی (P=0/001) و فاطمی (P<0/001) تفاوت معناداری داشت. بحث: نتایج مطالعه حاضر نشان داد که تعداد جراحی ها در طول همه گیری کووید 19 نسبت به مدت مشابه در سال قبل کاهش پیدا کرده بود و به نظر می رسد این امر به دلیل کاهش تعداد بستری های بیمارستانی با قصد عدم تحمیل بار اضافی به بیمارستان ها و کادر درمان و ایجاد امکان ارائه خدمات بیشتر به بیماران کرونایی بوده است.

Investigating the process of providing surgical services during the pre and post COVID-19 pandemic at the Dr.Fatemi and Imam Khomeini hospitals in Ardabil

English Abstract

Background: Globally, Covid-19 has put the health system under stress and has negatively affected the ability to provide complete and adequate health care and services to patients. In this study, we have examined the process of providing surgical services during the pre and post COVID-19 pandemic at the Dr.Fatemi and Imam Khomeini hospitals in Ardabil (year 2020 comparedto year 2019). Aim: To evaluate the process of providing surgical services during the pre and post COVID-19 pandemic at the Dr.Fatemi and Imam Khomeini hospitals in Ardabil. Materials and methods: In this analytical cross-sectional study, 1028 patients who underwent surgery in Ardabil Imam Khomeini and Fatemi hospitals during 2019 and 2020 (before and during the corona pandemic) were included in the study, and the required variables included the type of surgery, age, gender, weight, blood pressure, time interval from initial diagnosis to surgery, surgical service, and length of hospitalization were determined for all and recorded in pre-designed checklists. Results: The number of surgeries in 2020 compared to 2019 decreased by 59.8% and 11.4% in Imam Khomeini and Dr. Fatemi Hospitals, respectively. The frequency of surgeries in 2020 and 2019 had a significant difference by surgery service in both Imam Khomeini (P=0.011) and Fatemi (P=0.007) hospitals and also by type of surgery in both Imam Khomeini hospitals (P=0.001) and Fatemi (P<0.001) hospitals. Conclusion: The results of the present study showed that the number of surgeries during the covid-19 epidemic had decreased compared to the same period in the previous year, and it seems that this is due to the decrease in the number of hospitalizations with the intention of not imposing an additional burden on hospitals and health staff and making it possible to provide more services to corona patients.
