بررسی میزان بقا در سرطان پستان و بررسی برخی عوامل موثر بر آن در استان اردبیل

Persian Abstract

مقدمه وهدف : سرطان پستان شايع ترين سرطان در خانم ها و دومين علت مرگ ناشي از سرطان پس از سرطان ريه مي باشد.هدف از اين مطالعه تعیین میزان بقاء و برخی عوامل موثر بر آن در در بیماران مبتلا به سرطان پستان دراستان اردبیل می باشد. مواد و روش ها: مطالعه توصیفی تحلیلی ازنوع تحلیل بقاء بصورت گذشته نگر بر روی پرونده بيماران مبتلا كه از سال 1382 تا پايان سال 1387 در مرکز ثبت سرطان استان اردبیل وجود داشت با روش کاپلان مایر تجزیه و تحلیل داده ها انجام شد و اثر متغییر های سن ،جنس،محل تومور،محل سکونت،نوع درمان بااستفاده از مدل رگرسیونی کاکس وآزمون لگ رنک تعیین گردید. يافته ها: ميانگين (± انحراف معيا ر ) سن بيماران 4/45 (±28/12)بود ميزان بقاي کلی 5و4و3و2و1 ساله ی بيماران به ترتيب 51%،59%،67%،71%،82% درصدمحاسبه گردید.بااستفاده از مدل رگرسیونی کاکس متغییر های سن،درمان جراحی،دارو درمانی،محل سکونت برمیزان بقای بیماران تاثیر معنی داری داشتند. نتيجه گيری: میانگین سن تشخیص در اردبیل با سایر مطالعات انجام شده در سایر نقاط کشور تقریبا هماهنگ است،ولی نسیت به جوامع توسعه یافته2 دهه کمتر می باشد.ميزان بقاي 5 ساله ی بيماران در استان اردبیل پايين تر از سایر مطالعات انجام شده در کشور بود. هم چنين ميزان بقاي 5 ساله بيماري در كل نيز نسبت به اكثر مطالعات جهان كمتر بود.عدم تشخیص و مراجعه ی به موقع بيماران و عدم وجود امكانات درمانی مناسب در استان ازعلل ميزان بقاي پايين تر سرطان سینه در این استان مي باشند. لذا آموزش به خانم هادرجهت شناسايي سرطان پستان در مراحل اوليه و انجام برنامه های غربالگری مناسب بايد با جديت بيش تري توسط مراكز بهداشتي دنبال شود

Applied Survival Analysis in Breast cancer and Obtain Effective Factors On Breast Cancer In Ardebil Province

English Abstract

Introduction:breast carcinoma is the most common cancer in women and the second leading couse of cancer related mortality after lung cancer.in this study we estimated breast cancer survival rate and evaluated some effective factors on it. Materiales and methods:retrospective analytical study was carried on patients records from 1382-1387 ,that have been recorded in cancer registration center of Ardebil with Kaplan meier method for determine the survival rate of patients with breast cancer . effect of factors such as age,sex,morphology,residency place and type of treatment might effect on survival rate was studied . Results:mean age of breast cancer diagnosis was 45/5 years in Ardebil. survival rate for 1 year was83% for 2 year was 71% for 3 year was 67% for 4 year was 59% and for 5 year was 51%. Results showed that age,surgery, drug thrapy and residency place were associated with survival of breast cancer. Conclusion: mean age of breast cancer diagnosis in Ardebil was similar to other studies performed in Iran . in compared with developed countries it was 2 decade lower than .5-years survival in ardebil was lower than in other places of Iran and other countries.late diagnosis and lack of chemothrapy and radiotherapy sites may be causes of shorter survival in Ardebil. Therefore education for early diagnosis in women must be considered and these findings support the need for breast cancer screening programs.
