مقايسه اثر پروپوفول و تيوپنتال سدیم در القای بیهوشی برای الکتروشوک درمانی

زمینه و هدف: الکتروشوک درمانی يکی از روشهای رايج در درمان انواع بيماريهای روانی بوده و ميزان تاثير درمانی اين روش ارتباط مستقيمی با طول مدت تشنج دارد. تعيين ميزان تاثير داروهای بيهوشی روی مدت تشنج ناشی از ECT و همينطور جلوگيری از عوارض هموديناميک آن حائز اهميت است. در اين مطالعه ما دو داروی پروپوفول و تيوپنتال سديم را در القاء بيهوشی برای ECT مورد مقايسه قرار داديم. روش کار: در يک مطالعه آينده نگر، تصادفی و يک سويه کور تعداد 16 بيمار روانی در طی 72 جلسه ECT به صورت متناوب تحت بيهوشی با تيوپنتال سديم و سوکسينيل کولين و يا پروپوفول و سوکسينيل کولين قرار گرفته و مدت تشنج ظاهری و تشنج واقعی تائيد شده با Electroencephalography و تغييرات هموديناميک (تعداد ضربان قلب و فشار متوسط شرياني) و همچنين بروز عوارض مختلف ناشی از بيهوشی و ECT ثبت گرديد. نتايج توسط نرم افزارآماری SPSS ويرايش 9 تحت آناليز قرار گرفت. یافته ها: مدت تشنج ظاهری و تشنج واقعی ناشی از ECT (Electroconvulsive therapy ) در گروه پروپوفول کمتر از گروه تيوپنتال بود ولی از نظر آماری اين اختلاف معنی دار نبود (p=0.318). ميزان تغييرات مولفه های هموديناميک با پروپوفول کمتر بوده و ميزان افزايش فشار متوسط شريانی بعد از تجويز داروها و بلافاصله بعد از ECT اختلاف معنی داری در دو گروه داشت (p=0.04). ميزان شيوع ساير عوارض تنفسی و هموديناميک، تهوع، استفراغ و بيقراری در دو گروه اختلاف معنی داری نداشتند. نتيجه گيری: پروپوفول جهت القاء بیهوشی در ECT از نظر مدت تشنج، تفاوت بارزی با تیوپنتال سدیم ندارد. بنابراین از نظر بالینی می توان از آن به جای داروهای ديگر در ECT استفاده نمود.

Comparing the Effects of Propofol and Sodium Thiopental in the Induction of Anesthesia for Electro Convulsive Therapy (ECT)

English Abstract

Background & Objectives: ECT is one of the most common methods in treatment of different types of psychological disorder. The effectiveness of this therapy has direct relation to the duration of convulsion. Knowing the rate of efficacy of anesthetic drugs over convulsion by ECT and preventing of hemodynamic complications are important. This study was conducted to compare effect of Propofol with Sodium Thiopental in the induction of anesthesia during ECT. Methods: The subjects of this randomized, controlled and trial study were 16 patents with psychotic disorders. These patients received Sodium Thiopental and Succinylcholine or Propofol and Succinylcholine during 72 sessions of ECT. Duration of subjective and objective convulsion and hemodynamic changes (HR and MAP) as well as side effects of anesthesia and ECT was recorded. The collected data were analyzed by SPSS. Results: Duration of subjective and objective convulsion was lower in Propofol taken group than Sodium Thiopental taken patients, but this difference was not statistically significant (p=0.32). The variation of hemodynamic parameter was lower among Propofol taken patients. We observed significant difference between two groups regarding to increase of arterial BP’ mean after taking drugs and immediately after ECT (p=0.04). There was not significant difference from view points of prevalence of respiratory, Hemodynamic complications, nausea, vomiting and restlessness between two groups. Conclusion: Duration of convulsion in induction of anesthesia in ECT with Propofol and Sodium Thiopental was not different.
