بررسی ارتباط بین وضعیت قرار گیری مادر با نتایج آزمون بدون استرس در مادران حامله پرخطر بستری در مرکز آموزشی درمانی الزهرا ،تبریز در سال 1382

مقدمه: از آنجا که داشتن کودکی سالم از مهمترین اهداف مراقبتهای بارداری است، درجهت کاهش مرگ و میر و عوارض جنینی و نوزادی، انجام برخی ارزیابی های قبل از تولد نظیر: آزمون بدون استرس که یک تست غربالگری سلامت جنین است، ضرورت می یابند. مزایای این آزمون شامل، غیرتهاجمی بودن، بی دردی، بی خطری برای مادر، نداشتن مورد منع استفاده، کوتاهی زمان اجراء، قابلیت استفاده در بیماران سرپایی و بستری، سهولت اجرای آن و در دسترس بودن فوری نتایج می باشد. عوامل گوناگونی بر نتایج آزمون تاثیر می گذارند که یکی از آنها وضیعت مادر می باشد، به همین دلیل پژوهشگر بر آن شد تا به بررسی ارتباط بین وضیعت قرارگیری مادر با نتایج آزمون بدون استرس در زنان حامله پرخطر بستری در مرکز آموزشی درمانی الزهرا تبریز سال 1382 بپردازد . روش کار : این پژوهش یک کار آزمایی بالینی است که بر روی 150 مادر حامله پرخطر با حاملگی تک قلویی بین هفته های 32 تا 42بارداری انجام شد. برای انجام آزمون، اولین بیمار برای وضیعت نیمه نشسته و یا خوابیده به پهلوی چپ به طور تصادفی انتخاب شد و سپس مادران به ترتیب ابتدا در وضیعت نیمه نشسته و بعد در وضییعت خوابیده به پهلوی چپ یا بالعکس (در هر وضعیت به مدت 10 دقیقه ) تحت مونیتورینگ خارجی جنین قرار گرفتند. روش نمونه گیری غیر احتمالی مبتنی بر هدف (فرم انتخاب نمونه ) و روش جمع آوری داده ها به وسیله چک لیست بود. برای تجزیه و تحلیل از آمار توصیفی (میانگین ، انحراف معیار و فراوانی )و آمار تحلیلی (آزمون کای دو و آزمون مک نمار ) استفاده شد. نتایج: نتایج پژوهش نشان داد که بین وضیعت قرار گیری مادر و نتایج آزمون بدون استرس در سطح 05/0 ارتباط معنی دار وجود ندارد ولی تغییر پاسخ بدون عکس العمل به با عکس العمل در وضیعت اول خوابیده به پهلوی چپ سپس نیمه نشسته بیشتر بود. بحث: با توجه به نتایج پژوهش و با درنظر گرفتن این که وضیعت از عوامل موثر بر نتایج آزمون بدون استرس می باشد، استفاده از آزمون بدون استرس در هر دو وضیعت (نیمه نشسته و خوابیده به پهلوی چپ) پیشنهاد می شود.

The relationship between maternal position and results of nonstress test among high risk pregnant women hospitalized in Alzahra treatment - educational center of Tabriz in 2003

English Abstract

Introduction: Having a healthy child is one of the most important aims of pregnancy care. For decreasing fetus and infant morbidity and mortality rate, perinatal assessment such as nonsterss test is required. It is one of the screening tests for fetal surveillance. It’s advantages include: being noninvasive, painless, without risk to mother or fetus, no contraindication, short duration of conduction test, used for out and hospitalized patients, easy to do as well as availability of results immediately. Maternal position is one of the factors which affect the results of nonstress test. The relationship between maternal position and results of nonstress test was evaluated among high risk pregnant women hospitalized in Alzahra treatment-educational center of Tabriz in 2003. Material and Method: This is a clinical trial study in which 150 patients with singleton pregnancy at 32 to 42 week’s gestation. Sampling method was nonprobability purposive. Data gathering tools were checklist and sample select form. First patient was randomly assigned to a ‘sitting first’ (semi-Fowler position) or ‘supine first’ (left lateral position) and then position was alternated. Fetal heart rate monitoring was performed for ten minutes in each position. For data analysis, descriptive statics(means, standard deviation) and analytic statics(chi-squared test, McNemar test) were used. Results: There was no significant correlation between maternal position and results nonstress test (p=0.05). Conclusion: According to the results, using of both positions for performing nonstress test is suggested.
