مقایسه تأثیر شیاف واژینال لاکتوباسیلوس اسیدوفیلوس با قرص واژینال مترونیدازول در تنظیم pH واژن مبتلایان به واژینوز باکتریال

دراین مطالعه مقایسه تأثیر شیاف واژینال لاکتوباسیلوس اسیدوفیلوس با قرص واژینال مترونیدازول در تنظیم pH واژن مبتلایان به واژینوز باکتریال انجام شد. مواد و روشها:این مطالعه کارآزمایی بالینی تصادفی و دوسوکور بود که در مراکز درمانی سرپایی عمومی و خصوصی دانشگاه علوم پزشکی تبریز در سال 1387 انجام شد. از تعداد 300 بیمار، 40 بیمار مبتلا به واژینوزباکتریال طبق معیارهای امسل تشخیص داده شدند و بصورت تصادفی 20 بیمار در گروه شیاف لاکتوباسیل و 20بیمار در گروه قرص واژینال مترونیدازول قرار گرفتند. مدت مصرف داروها،7شب تعیین شد. یافته ها: نتایج قابل توجهی بین گروه قرص واژینال مترونیدازول و گروه شیاف لاکتوباسیل در کاهش pHواژن وجود نداشت.( p=0/118). با وجود این میانگین اختلاف pH واژن در گروه مترونیدازول قبل و بعد از مداخله 17/1 ±12/1،(p<0.001) و در گروه لاکتوباسیل ,76/0 ± 62/1 (p<0.001) بود. بحث و نتیجه گیری: مطالعه نشان داد که آزمایش روتین pH می تواند تشخیص مبتلایان به واژینوز باکتریال را افزایش دهد و متعاقب آن توسط روشهای آزمایشگاهی نیز تائید گردد و شیاف واژینال لاکتوباسیل در مقایسه باقرص واژینال مترونیدازول در کاهش pH واژن موثرتر است.

Comparison between Effectiveness of Lactobacillus Acidophilus Suppository and Metronidazole Vaginal Tablet with Aim of Regulation of Vaginal PH in Patients with Bacterial Vaginosis

Background & Objectives: Increase in vaginal pH is an indicator of bacterial vaginosis , changing a healthy acidic environment to a more acidic than 4.5. This change is due to an overgrowth of anaerobic bacteria and a decrease in the number of H2O2-producing lactobacilli;subsequently there will be normal vaginal ecosystem. Resistance to antibiotics has led to increasing consideration of non-antibiotic treatments. Two non-biotic methods of bacterial vaginosis therapies consist of (1) probiotic therapies, which aim to provide the bacteria as the direct supplement and restoring a women healthy lactobacillus, (2) acid therapies such as buffer gel. This study aims to compare the effectiveness of Lactobacillus acidophilus suppository and metronidazol vaginal tablet in order to regulate the vaginal pH in patients with bacterial vaginosis. Materials & Methods: A double-blind, randomized controlled trial was conducted in Outpatient Health Centers of Tabriz University of Medical Science (2008). 40 out of 300 enrolled patients were diagnosed with bacterial vaginosis. 20 ones of them were randomly put in metronidazol vaginal tablet group and the other 20 patients were put in Lactobacillus acidophilus suppository group. The trial period was 7 nights. Results: There were no significant differences in pH decreases between the metronidazol and lactobacilli vaginal groups (p=0.118); however the mean difference of vaginal pH in the metronidazol group before and after treatment was (1.12±1.17, p<0.001) whereas it was (1.62±0.76, p<0.001) in the lactobacilli group. Conclusion: This study shows that routine pH testing can increase detection of bacterial vaginosis and promote laboratory diagnosis of it. The lactobacilli suppository was more effective than metronidazol vaginal tablet in decreasing vaginal pH in affected ones.
