بررسی تاثیر کپسول هیدرکسی اوره در درمان بیماران تالاسمی اینترمدیایی که به صورت ماهیانه خون دریافت کرده اند.

Persian Abstract

مقدمه و هدف: بتاتالاسمی ماژور یکی از شایعترین اختلالات ژنتیک در جهان می باشد . افراد هموزیگوت گاهی فرم خفیف تری از این بیماری را نشان می دهند که به آنها اینترمدیا اطلاق می شود. بعضی داروها مانند هیدروکسی اوره در مبتلایان به تالاسمی امتحان شده است. در این مطالعه، اثر هیدروکسی اوره در بیماران تالاسمی اینترمدیایی که بعنوان تالاسمی ماژور با تزریق خون در درمانگاه تالاسمی بیمارستان بوعلی اردبیل تحت درمان بوده اند، بررسی شد. مواد و روشها: با بررسی پرونده بیمارانیکه به عنوان تالاسمی ماژور تحت درمان با packed-Cell بودند، بیمارانیکه اولین تزریق خونشان بعد از 2 سالگی بوده و یا هموگلوبین اولیه بالای 7 داشتند، بعنوان تالاسمی اینترمدیا در نظر گرفته شده و بعد از کسب رضایت نامه کتبی تزریق خون آنها قطع و روی درمان با هیدروکسی اوره قرار گرفتند. بعد از 6 ماه، نیاز آنها به تزریق خون بررسی شد. در این مدت میزان هموگلوبین، فریتین، سطح سلولهای خون و عملکرد کلیه و کبد قبل و بعد از هیدروکسی اوره ارزیابی و نتایج حاصل از این بررسی هادر نرم افزار SPSS مورد آنالیز قرار گرفت. نتایج: در این مطالعه 63 بیمار، که 45 نفر مرد(%71.4)و 18 نفر زن(%28.6)، با دامنه سنی 10-40 سال و میانگین سنی 22.2 سال بودند، بررسی شدند. بعد از 6 ماه، 60 نفر(95.2 %)بطور کامل از تزریق خون بی نیاز شدند(P-value = 0.001)، که از نظر آماری معنی دار بوده و 3 نفر(4.8%) فواصل تزریق خونشان افزایش پیدا کرد(P<0.05)، که معنی دار بوده است. میانگین هموگلوبین بعد از مصرف دارو افزایش(P-value=0.001) و میانگین فریتین کاهش داشت(P-value=0.001). از عوارض دارو، افزایش AST (P<0.05)، ALT (P<0.05)و BUN(P<0.05) معنی دار بوده است. بحث: درمان با هيدروكسي اوره در بيماران بتا تالاسمي اينترمديا، باعث برطرف شدن نیاز به تزریق خون و افزايش سطح هموگلوبين در آنها می شود.

Evaluation of benefit of Hydroxy Urea capsule in treatment of patients with thalassemia intermedia who treat with monthly packed cell transfusion.

English Abstract

Introduction: The β-thalassemia is one of the most common genetically transmitted blood disorders in the world. Sometimes Homozygote people express the some milder form of disorder called intermediate. Some drugs like HU were tested in treatment of thalassemia patients. In this study the effect of HU in intermediate patients who missed as major thalassemia and treated monthly by packed cell transfusion in BooAli Hospital of Ardebil was evaluated. Material and method: By investigating the thalassemia major patient’s files, those who had their first transfusion above 2 years old or their Hb before first transfusion was above 7mg/dl, considered as Intermedia. And all transfusions discontinued and patients treated by HU for 6 months. Before starting hydroxyurea and after 6 month of treatment, all patients underwent CBC, Serum ferritin, liver and kidney function tests. At the end of 6 months the patient need for transfusion and laboratory tests results were analyzed through SPSS. Results: 63 patients enrolled in the study. The study group consists of 45 men and 18 women. The average age was 22.2 years (10-40 years). After 6 month, 60 of 63 patients became completely transfusion free (p-v=0.001), in 3 patients transfusion interval prolonged more than one month (p-v<05). We observed a decrease in serum ferritin level (p-v=0.001), and an increase in hemoglobin level (p-v=0.001). Also Rise of AST, ALT and BUN also seen after treatment with HU (p-v<0.05). Discussion and conclusion: In thalassemia intermediate patients HU can obviate the need for regular transfusion and increase mean Hb.
