بررسی شاخص های نسخه پپچی در داروخانه های شهر اردبیل از مهر 1387 تا اردیبهشت 1388

مصرف دارو فرآیند پیچیده ای است که پزشک،داروساز و بیمار در چگونگی آن دخیل اند. تعدادی شاخص توسط سازمان جهانی بهداشت برای ارزیابی جنبه های مختلف تجویز و مصرف منطقی دارو طراحی شده اند. این شاخص ها در سه دسته تحت عنوان شاخص های تجویز، مراقبت از بیماران و مراکز درمانی طبقه بندی می شوند. شاخصهای نسخه پیچی یک دسته از شاخص های مربوط به مراقبت از بیماران می باشند که خود شامل متوسط زمان (ADT)Dispensing ، درصدی از داروهای تجویز شده که تحویل بیمار شده اند (PDAD)، درصدی از داروها که نحوه مصرفشان مکتوب شده است(PDAL) و میزان اطلاعات بیمار از دوز مصرف دارو PKDD می باشد. ما شاخصهای فوق را به طور آینده نگر در یک دوره هشت ماهه از مهر 1387 تا اردیبهشت 1388 اندازه گیری و بررسی کرده ایم. تعداد نسخ مورد مطالعه 736 عدد بوده اند. نسخ مورد مطالعه به روش نمونه گیری تصادفی سیستماتیک از 26 داروخانه خوشه گیری شده از 60 داروخانه شهر اردبیل انتخاب شدند. کارشناسان پرسشگر طرح، با مراجعه به داروخانه ها زمان Dispensing را به طور نامحسوس توسط کرونومتر اندازه گیری و در پرسشنامه ثبت نمودند. سایر اطلاعات مربوط به شاخصهای نسخه پیچی پس از رویت نسخه ها و داروهای نسخه پیچی شده و مصاحبه با تحویل گیرنده داروها توسط پرسشگران بدست آمده و در پرسشنامه ثبت شده اند. متوسط شاخص های نسخه پیچی بدست آمده به ترتیب زیر بوده اند: ADT=20/7 PDAD=98/5% PDAL=85/3% PKDD=79/7% زمان 7/20 ثانبه بدست آمده با توجه به اینکه متوسط تعداد قلم داروهای تجویزی در هر نسخه 6/3 بوده در مقایسه با استاندارد سازمان جهانی بهداشت (3 دقیقه برای 5/1 الی 2 قلم دارو در هر نسخه) زمان کوتاهی می باشد و زمان بیشتری بری توضیح رژیم دارویی، احتیاطات ضروری و عوارض جانبی دارویی لازم است. زمان کوتاه بدست آمده ضرورت ادامه برنامه های مربوط به بهبود تجویز و مصرف دارو مخصوصاً تلاش برای اصلاح دید اقشار مختلف جامعه به داروخانه ها بعنوان یک مرکز علمی و دارویی را ایجاب می کند. واژه های کلیدی: شاخص های نسخه پیچی ، دارو، داروخانه

Evalution of dispensing indictors in pharmacies of Ardabil from September of 2008 to April of 2009.

English Abstract

Drug use is a complex subject involving the physician, the patient, and pharmacists. A number of indicators have been developed, standardized and evaluated by world health organisation(WHO), which provide useful means of measuring certain aspects of countries drug use. These indicator are grouped into three categoris: prescribing, patient care and facility indicators. Patient care indicators in pharmacies are: Average Dispensing Time(ADT), Percentage of Drug Actually Dispensed (PDAD), Percentage of Drug Adequately Labelled (PDAL) and Patient's Knowledg of the correct Drug Dose (PKDD). We prospectively studied these indicators in pharmacies of Ardabil over a eight months period. Volume of our study was 736 prescritions. Samples of study were selected by systematic random sampling from 26 pharmacies of city from September of 2008 to April of 2009. Relevant data were recorded on forms which were developed and designed by WHO. ADT was measured by stopwatch and other indicators were recorded after observation of dispensed drugs of patients which leaved the pharmacies and by questionnaire. The means of indicators were calculated: the ADT was 20/7 Seconds (+12). The PDAD was 98.5 , the PDAL was 85.3 and the PKDD was 79/7. The ADT in our investigation(20/7 Seconds for3/6 number of drug prescribed) is very short time when compared to WHO standard.(3 minutes for 1.5-2 number of drugs prescribed). More time is needed between pharmacist and patient, to explain the dosage regimen , necessary precautions, and adverse drug effects. Short ADT and results highlight necessity of continuing the process of rationalizing therapeutic drug use in Ardabil, encouraging therapeutic drug use in Ardabil, encouraging pharmacist_patient interaction, and providing training in the rational use of drugs.
