مقايسه سطح آنزيم هاي كبدي پرسنل اتاق عمل با كادر درماني خارج از اتاق عمل

Persian Abstract

زمینه و هدف: تماس طولانی مدت با گازهای استنشاقی بیهوشی به ویژه هالوتان به عنوان یک خطر جدی برای سلامتی کادر اتاق عمل محسوب می­گردد. این مطالعه با هدف مقایسه سطح آنزیم­های کبدی پرسنل اتاق عمل با کادر درمانی خارج از اتاق عمل انجام گرفت. مواد و روش­ها: مطالعه از نوع مقطعی-تحلیلی می باشد،180 نفر از پرسنل بیمارستان­های آموزشی اردبیل وارد طرح شدند و اطلاعات دموگرافیک و سوابق کاری در پرسشنامه جمع­آوری شد. سپس تست­های کبدی و بیلی­روبین این افراد مورد سنجش قرار گرفت و در موارد با مقادیر آنزیمی بالاتر از حد نرمال، به منظور رد هپاتیت B و C تست هایHCV-Ab ,HBS-Ag ,HBS-Ab و برای رد کبد چرب سونوگرافی کبدی انجام گرفت. بر اساس معیارهای ورود و خروج، اطلاعات مربوط به تست­های کبدی 100 نفر (53 نفر پرسنل اتاق عمل و 47 نفر پرسنل خارج از اتاق عمل) با آزمون آماری مورد تجزیه وتحلیل قرار گرفت. یافته ها: مشخصات دموگرافیک و مدت اشتغال در دو گروه همسان بود. میانگین آنزیم­های AST,ALT در کادر اتاق عمل به طور معنی­داری بالاتر از کادر خارج اتاق عمل بود(05/0>P) و میانگین آنزیمهای ALP,GGT و بیلی­روبین در دو گروه اختلاف معنی­داری نداشت(05/0<P). ضمناً میزان آنزیم­های GGT,AST,ALT با افزایش سن پرسنل اتاق عمل (بالای 40سال) و سابقه کاری بالا (بیش از 15 سال) به طور معنی­داری افزایش نشان داد. نتیجه­گیری: نتایج نشان داد پرسنل اتاق­های عمل مراکز آموزشی درمانی احتمالا در معرض عوامل هپاتوتوکسیک (هالوتان و N2O) قرار دارند، که منجر به افزایش معنی­دار آنزیم­های کبدی در آنها گردیده است.

Comparison of hepatic enzymes level between operating room's staff and other wards’ personnel

English Abstract

Introduction: Long exposure to inhalant anesthetic gases especially Halothane among the known hepatotoxic agents, are as the serious health risks for those working in operation room. Considering importance of risks of working in operating room and exposure to anesthetic gases especially Halothane that is used extensively in our country and non standard air conditioning systems of operation room of Ardebil’s medical and educational centers, current study was performed to compare level of hepatic enzymes between personnel of operation room and other therapeutic wards’ personnel (out of operation room) in Ardabil Medical University hospitals. Materials and Methods: In this Analytical Cross-sectional study, 180 subjects of educational hospital’s personnel were included, their demographic and occupational information were recorded. Then hepatic function tests (ALT, AST, GGT, ALP and bilirubin) were analyzed and in the cases with values more than normal, to rule out Hepatitis (Band C) Hbs-Ag, Hbs-Ab and HCV-Ab were tested and to rule out Fatty liver Sonography was performed. On the basis of inclusion and exclusion criteria data of hepatic tests of 100 persons (53 of operating room personnel and 47 of other parts personnel) were analyzed with analytical statistic test, t-test. Results: Demographic information and employment span were alike. The average of ALT, AST were higher among personnel of operating room in comparison with personnel of other parts with a significant difference (PV<0.05) and average of GGT and ALP didn’t have significant difference between two groups (PV>0.05) Also values of ALT, AST and GGT indicated a significant increase with the age of personnel (more than 40 years) and increasing of employment period (more than 15 years) Discussion and Conclusion: Results of this study show that personnel of operation rooms in educational hospitals of Ardabil, may be subject to hepatotoxic agents (Halothane and N2o) that cause significant rise of hepatic enzymes It is recommended to use appropriate anesthetic methods besides good air conditioning systems are required
