بررسی شاخصهای دارویی و میزان خطاهای نسخه نویسی در نسخ بیمه ای دندانپزشکان شهرستان اردبیل در سال 1391

سابقه و اهداف : برای تسریع روند درمان و بهبودی بیمار و نیز جهت پیشگیری از عواقب نامطلوب پزشکی،دندانپزشکان باید از خطاهای نسخه نویسی مانند شکل دارویی،دوز،تداخلات دارویی و ... آگاه باشند.هدف از مطالعه حاضر بررسی خطاهای نسخه نویسی و تعیین شاخص های دارویی دندانپزشکان شهر اردبیل بوده است. مواد و روش ها : در این مطالعه توصیفی مقطعی تعداد 1000نسخه بیمه خدمات درمانی بصورت تصادفی از میان نسخ تجویز شده توسط دندانپزشکان شهر اردبیل در یک دوره 1 ساله انتخاب شده و بصورت گذشته نگر از نظر وجود خطا در شکل دارویی ،نام دارو ،دوز دارو ،فاصله دوزها ،روش تجویز ،تعداد اقلام هر نسخه و تداخلات دارویی و خوانا یا ناخوانا بودن نسخ مورد ارزیابی قرار گرفتند و در نهایت اطلاعات جمع آوری شده از نسخ در نرم افزار آماری SPSS وارد و با استفاده از روشهای آمار توصیفی وتحلیلی آنالیز گردید. یافته ها : از کل نسخ مورد بررسی 71% دارای خطا بودند. خطا در نوشتن نام دارو در 42% نسخ، میزان خطا در نوشتن روش تجویز در 15% نسخ ، خطا در نوشتن شکل دارویی در12% نسخ ،خطا در نوشتن دوز دارو در 2% نسخ، ، خطا در نوشتن فاصله مصرف دوزها (interval) در 29% نسخ، و وجود تداخلات دارویی در0% نسخ مشاهده شد. نتیجه گیری : با توجه به این که اکثر نسخ بررسی شده دارای خطا بودند ،پیشنهاد می شود در برنامه های درسی رشته دندانپزشکی و نیز در برنامه های مدون بازآموزی دندانپزشکان بر اصول صحیح نسخه نویسی تاکید بیشتری گردد.ضمنا با در نظر گرفتن کسوراتی برای نسخ دارای خطا می توان دندانپزشکان را برای رعایت اصول صحیح نسخه نویسی ترغیب نمود.

Assessment of Drug Indices & Prescribing Error Rates in Insurance Prescriptions of Ardabil City Dentists In 1391

English Abstract

Background and objectives:In order to improve the process of treatment and preventing adverse consequences. Dentists should be informed about prescribing errors such as type of medicine, dose, medicine interactions, etc. The aim of this study was assessment of drug indices and prescribing error rates in insurance prescriptions of Ardabil city dentists. Materials and Methods:In this descriptive study insurance number of1000 prescriptions which were written by dentists in Ardabil city during 1 year were selected randomly and evaluated as retrospective study considering errors of medicine type, name of medicine, dose, intervals, way of prescribing, number of medicines in each prescription, medicine interactions and legibility of prescriptions and then with data collected from SPSS and analyzed using descriptive statistics and analysis. Results:71% of studied prescriptions had errors.Type of medicine, name of medicine, way of prescribing, time between doses and doses had been written wrong in 42%, 15%, 12%, 2%, 29% of prescriptions, respectively.0% of prescriptions had medicine interactions among prescribed medicines. Conclusion:Regarding high percentage of errors among studied prescriptions, it is suggested to emphasis more on principles of prescription at university and retraining courses for dentists. It is possible to encourage them by reducing some money given for each wrong prescription.
