بررسی شاخصها و الگوی تغييرات اپيدميولوژيکی در بيماران مبتلا به ليشمانيوز احشايی در مراکز بهداشتی-درمانی شهرستان مشکين شهر طی سال های 1380 تا 1391: (يک مطالعه گذشته نگر)

Persian Abstract

زمينه و هدف: ليشمانيوز احشائی از بيماريهای مهم آندميک در ايران است و شهرستان مشکين شهر مهمترين کانون طبيعی ليشمانيوز احشايی در ايران می باشد. هدف از اين مطالعه بررسی آخرين تغييرات در الگوها و شاخصهای اپيدميولوژيکی بيماری کالاآزار در منطقه مشکين شهر طی 12 سال می باشد. روش کار: اين پژوهش توصيفی - تحليلی به صورت گذشته نگر بوده و با بررسی پرونده 193 بيمار که در فاصله زماني1380 لغايت 1391با تشخيص کالا آزار در بيمارستان وليعصر مشکين شهر بستری بوده و يا در منزل تحت درمان بوده اند، صورت گرفته است. اطلاعات از پرونده ها استخراج و داده ها با استفاده از آزمونهای آماری توصيفی و به کمک نرم افزار SPSS نسخه 15 مورد تجزيه و تحليل قرار گرفتند. يافته ها: از 193 نفر بيمار کالا آزار تاييد شده از سال 1380-1391، 85% ساکن مناطق روستايی و 15% ساکن مناطق شهری بودند. 43% از بيماران مونث و 57% مذکر بودند. از نظر گروه سنی 66% زير 2 سال و 23% گروه سنی 2-5 سال و 11% بالای 5 سال بودند. در بين چهار بخش شهرستان، بيشترين موارد بيماری در پنج سال اول مطالعه در بخش مرکزی و در شش سال دوم در بخش غربی گزارش شده است و در يک سال اخير (1391) اين الگو مجددا" در حال تغيير می باشد. نتيجه گيری: بروز بيماری در منطقه مورد مطالعه کاهش يافته است. به علت آلودگی نسبتا" بالای سگ سانان و نقش مهم اين مخازن در حفظ و انتشار بيماری، توقف اقدامات بهداشتی در هر يک از بخش های منطقه می تواند باعث افزايش موارد بروز بيماری بشود.

Study on Changes in Epidemiological Patterns and Parameters of Visceral Leishmaniasis in Patients Referred to Health Care Centers of Meshkin Shahr during 2001-2012: (A Retrospective Study)

English Abstract

Background & Objectives : Visceral leishmaniasis is an important endemic disease in Iran and Meshkin shahr County is the most important foci of visceral leishmaniasis in Iran. The aim of this study is the survey of recent changes in epidemiological patterns of visceral leishmaniasis in Meshkin Shar area during 12 years. Methods : This is a descriptive retrospective study. Records of 193 cases with confirmed kala-azar disease were reviewed in the period of 2001-2012. Data collected from files and were analyzed using descriptive statistics and SPSS v.15. Results : From 193 patients with confirmed kala-azar disease, 85% were resident in rural and 15% in urban areas. Among of these patients, 43% were female and 57% male. About 66% of patients were under 2 years old, 23% in age group between 2-5 years and 11% above five years. Among four parts of Meshkin Shahr County, in the first five years of the study, the most cases of the disease were from the central part, but in second six years the most cases were from the west part of this county and in latest year (2012) it has been coverted . Conclusions : Incidence of kala-azar was decreased during last 12 years. Because of high infection rate in dogs and considering important role of these reservoirs in conserving and distribution of disease, stopped health activities in any parts of area can increase the incidence of disease.
