بررسي واسكوليت CNS كودكان در درمانگاه روماتولوژي اطفال

مقدمه و بیان مسئله: واسکولیت CNS، التهاب رگ های خونی در مغز است. به صورت یک بیماری خود ایمنی بوده که سیستم ایمنی بدن، سلول های طبیعی در عروق خونی مغز را به عنوان سلول مضر شناخته و به آن ها حمله می کند. یافته های اپیدمیولوژیک، بالینی و آزمایشگاهی باعث افزایش دقت تشخیص این عارضه گردیده است. با این حال چالش های بسیاری به تشخیص و درمان این بیماری در اطفال باقی مانده است.از این رو در این مطالعه به بررسی کودکان مبتلا به واسکولیت CNS پرداختیم. مواد و روش ها : مطالعه حاضر از نوع توصیفی می باشد. در این مطالعه اطلاعات پرونده تمامی کودکانی که با تشخیص واسکولیت CNS به درمانگاه روماتولوژی اطفال مراجعه کرده اند را انتخاب و اطلاعات موجود در پرونده از جمله اطلاعات دموگرافیک و نتیجه MRA بیماران را وارد چک لیست هایی که از قبل طراحی شده است کردیم. در انتها داده ها وارد برنامه آنالیز آماری شده و به توصیف داده ها پرداختیم. پس از تکمیل چک لیست های مذکور اطلاعات حاصل از آنها وارد SPSS v16 شده و با استفاده از روش های آمار توصیفی در قالب نمودار و جدول بیان شدند. نتایج : در این مطالعه میانگین سنی بیماران 93/3 ± 8 سال و 55 درصد دختر بودند و جهت تشخیص بیماران از MRI و MRA استفاده گردید. میانگین سن شروع علایم 21/3 ± 63/5 سال بود. شایع ترین علامت سردرد بود. 63/63 درصد کودکان مبتلا به cPCNS درگیری عروق ریز داشتند و 63 درصد این بیماران درگیری ACA و MCA را نشان دادند. از میان این بیماران 27/77 درصدشان به درمان تجویز شده پاسخ خوب(Good) دادند. نتیجه گیری: نتایج نشان داد که واسکولیت CNS در کودکان عمدتا عروق ریز و شریان های قدامی و میانی مغز را درگیر می کند و اکثریت بیماران به درمان پاسخ خوبی می دهند.

Evaluation Of Central Nervous System Vasculitis In Children Pediatric Rheumatologic Clinic

English Abstract

Introduction : Vasculitis CNS is inflammation of the blood vessels in the brain. It is as an autoimmune disease in which the immune system detects normal cell in the blood vessles of brain as a harmful cells and attacks to them. The epidemiological, clinical and laboratory findings has increased accuracy of its diagnosis. However, many challenges has been remained in diagnosis and treatment of this disease in children. Hance this study examind children with CNS vasculitis. Material and Method : There is a descriptive study. In this study were selected information of all children with CNS vasculitis who referred to pediatric rheumatology clinic and also demographic and MRA findings were entered into the check lists which had been designed previously. After completing check lists, obtained data were entered into the SPSS v16 statistic software and by using descriptive statistics were expressed in the form of charts and tables. Results : In this study the average age of patients were 8±3.93 years, 55% were girl, MRI and MRA were used to diagnose patients. The average age of onset symptoms, was 5.63±3.21 years. The most common symptoms was headache. 63.63% of children with cPCNS had small vessel involvement and 63% of these patients showed involvement of ACA and MCA. Among them 77.27% responsed good to the treatment. Conclusion : Results showed that CNS vasculitis in children mainly involved small arteries and anterior and middle cerebral arteries and majority of patients respond good to treatment.
