بررسی تاثير فشار بر نقاطSP6 و LI4 بر شدت درد و طول مدت زايمان طبیعی در زنان نخست زا در بيمارستان هاي دولتي شهر اردبيل

زمينه و هدف: زايمان از جمله وقايع مهمي است كه اکثریت قریب به اتفاق زنان در زندگي تجربه كرده و درد جز اجتناب ناپذير آن است كه در صورت عدم كنترل منجر به طولاني شدن زايمان و بروز صدمات متعدد در مادر و جنين مي¬گردد لذا دستيابي به تدابيري جهت تسكين درد و كنترل طول مدت زايمان خصوصا با استفاده از روش هاي غيرتهاجمي نظير طب فشاري ضروري به نظر مي رسد. اين پژوهش به منظور تعيين تأثير فشار در نقاط SP6و LI4 بر شدت درد و طول مدت زايمان در زنان نخست زا انجام گرفته است. روش كار: اين مطالعه كارآزمايي باليني كنترل شده تصادفي بر روي 84 نفر از زنان نخست زا، در بيمارستان هاي دولتی شهر اردبيل انجام شد. شركت كنندگان به روش تصادفی توسط بلوك بندي 4 و 6 تايي به دو گروه تقسيم شدند. مداخله بصورت اعمال فشار در نقاط SP6 و LI4 در ديلاتاسيون هاي مختلف بود. شدت درد قبل و بعد از مداخله با استفاده از مقياس بينايي سنجش درد و طول مدت فاز فعال و مرحله دوم زايمان از طريق معاينه واژينال محاسبه و در چك ليست ثبت شد. براي تعيين تأثير فشار بر شدت درد و طول مدت زايمان از آزمون هاي آماري تي زوج، تي مستقل و كاي دو در نرم افزار SPSS / Ver. 16 استفاده شد. يافته ها: ميانگين شدت درد بعد از هر بار مداخله در گروه آزمون كاهش يافت و ميانگين شدت درد بعد از مداخله در گروه آزمون در هر چهار ديلاتاسيون مورد بررسي به طور معني دار كمتر از گروه كنترل بود (001/0p<)، همچنين ميانگين طول مدت فاز فعال و مرحله دوم زایمان در گروه آزمون به طور معنی دار کمتر از گروه کنترل بودند (001/0 P < ، 008/0 P =). نتيجه گيري: فشار بر نقاط SP6و LI4 در ديلاتاسيون هاي مختلف باعث كاهش شدت درد و طول مدت فاز فعال و مرحله دوم زايمان مي گردد. لذا اين روش آسان، غير تهاجمي و بي¬خطر مي¬تواند بطور مؤثري براي مددجويان در بالين و همچنين در حيطه¬هاي آموزشي و پژوهشي مورد استفاده قرارگيرد.

Effect of Acupressure at the SP6 and LI4 Points on the Intensity of Labor Pain and Delivery Length

English Abstract

Background and Objectives: delivery is one of the most important events in a woman's life, and pain is an inevitable part of childbirth. If the pain is not controlled, leads to prolonged labor and multiple injuries to mother and fetus. This research was done to determine the effect of acupressure at the SP6 and LI4 points on the duration and pain of the labor in nulliparous women. Methods: This randomized controlled clinical trial was conducted on 84 nulliparous women who were attended hospital in Ardabil. The participants were divided into two intervention and control groups using randomized blocking. Pressure have been done on SP6- LI4 points in intervention group. Pain intensity was measured before and after the intervention using visual analog scales and length of active phase and second stage were recorded by researcher vaginal exam in the checklist. Determining the effect of pressure on the intensity of labor pain and delivery length, we used Paired t-test, t-test and chi-square in spss v.16. Results: After the intervention mean of pain intensity in the intervention group were decreased. Also, there was a significant decrease in mean intensity of pain after each intervention in the intervention group with different dilations (4, 6, 8, and 10 cm) (p<0/001).The mean duration of active phase and second stage in intervention group were significantly less than the control group. (p<0/001, p=0/008). Conclusion: Since acupressure at the SP6- LI4point in different dilatations leads to the reduction in labor pain intensity and delivery duration, therefore, this method can be used to reduce the pain and duration of labor.
