جراحی در اسپونديلوآرتروپاتی گردن

Persian Abstract

پيش‎زمينه: سايش و گسيختگی در ديسک بين مهره‎ای و مفاصل فاست گردن در صورت علامت‎دار بودن، عدم جواب به درمان‎های غيرجراحی به آزادسازی نخاع گردنی و فيوژن مهره‎ها نيازمند هستند. اين گزارش از تجربه جراحی سائيدگی مهره‎های گردنی است. مواد و روش‎ها: در يک مطالعه مقطعی ۷۵ بيمار (۴۰ مرد، ۳۵ زن) مبتلا به آرتروز مهره‎های گردنی که بين سال‎های ۱۳۸۸ تا ۱۳۹۱ در دو بيمارستان اردبيل تحت عمل جراحی قرار گرفته بودند، به صورت گذشته‎نگر با پيگيری سه ماهه (۱ تا ۳ ماه) بررسی شدند. ميانگين سنی بيماران ۱۴/۴±۵۲/۳ سال بود. اطلاعات بيماران يا از طريق پرونده يا تماس تلفنی جمع‎آوری شد و مورد تحليل آماری قرار گرفتند. بهبود علايم عصبی و رضايت بيماران براساس معيارهای «ديداری درد» و «اودم» اندازه‎گيری شدند. يافته‎ها: در ۶۷ بيمار (۸۹/۳%) عمل جراحی با رويکرد جلويی و گذاشتن «کِيج» و در ۸ بيمار (۱۰/۷%) درمان با رويکرد پشتی انجام شده بود. بيشترين سطح درگيری (۳۳ بيمار،۴۴%) C5-C6 و ۴۳% بيماران (۳۳ نفر) رضايت کامل داشتند. عارضه آسيب نخاعی در يک مورد و دررفتگی گرافت در يک مورد مشاهده شد. نتيجه‌گيری: بيماران جراحی شده برای اسپونديلوآرتروپاتی گردن در کوتاه مدت، ميزان رضايت‌مندی و بهبودی بالايی از عمل جراحی داشتند.

Surgical Results of Cervival Spondiloarthropathy

English Abstract

Background: Wear and failure in inter-vertebral discs and facet joints with certain radiographical properties are called “cervical spondilosis”. The aim of this study was to evaluate neck spondiloarthrophaty and surgical results in patients referred to Ardabil city hospitals. Methods: A cross-sectional study was done on 75 patients (40 male, 35 female) with neck spondiloarthropathy referring to two hospitals in Ardebil-Iran from 2008 to 2012. The mean age of patients was 52.3±14.4 years old. A part of data was extracted from patient’s files and another part was gathered by telephone call. The data was analyzed by statistical methods. Improvement of neurological symptoms and patient satisfaction were measured with “Visual analogue scale” (VAS) and and Odom scale. Results: The majority received anterior surgery and “cage” implantation (89.3%) and 8 (10.7%) of patients received posterior release and fusion. 43% of patients were highly satisfied. Maximum involvement level in 33 cases (44%) was C5- C6 level. Conclusions: High level of satisfaction and improvement in early stage is possible in cervical spondyloarthropathy surgery.
