بررسی ارتباط بین نسبت بیلی روبین به آلبومین سرم در روز اول بستری نوزاد مبتلا به زردی با پاسخ به درمان

سابقه و هدف: زردی یكی از شایعترین نشانه های بالینی در نوزادان است. خطر زردی ناشی از احتمال بروز سمیت عصبی ناشی از بیلی روبین می باشد كه به آن کرن ایکتروس اطلاق می شود که ناشی از بیلی روبین غیر مستقیم آزاد می باشد .این مطالعه طراحی شده است تا با اندازه گیری نسبت بیلی روبین به آلبومین ارتباط آن با میزان پاسخ به درمان مورد ارزیابی قرار گیرد. مواد و روش ها: در این طرح تحقیقاتی 100 نوزاد ترم سالم با هیپر بیلی روبینمی بستری در بیمارستان بوعلی به این مطالعه وارد شدند. پس از جلب رضایت والدین، أخذ شرح حال و معاینه فیزیکی نمونه خون برای بیلی روبین توتال و مستقیم و آلبومین ارسال شد. پس از کامل کردن پرسشنامه ی مربوطه داده ها در نرم افزار آماری SPSS نوزده تجزیه و تحلیل شده و با استفاده آزمون t و ضریب همبستگی پیرسون(r) توصیف شدند. یافته ها: اختلاف معنی داری بین آلبومین در گروههای درمان شده وجود ندارد( 0.1± 4.6 ) میانگین نسبت بیلی روبین به آلبومین در گروهی که کمتر از 48 ساعت به درمان نیاز داشتند 3.6 و در گروهی که بیشتر از 48 ساعت به درمان نیاز داشتند 4.1 بود. ارتباط نسبت بیلی روبین به آلبومین با بیلی روبین توتال پس از درمان به صورت 42%r= می باشد (001/0pvalue=). ارتباط بیلی روبین توتال قبل از درمان با بیلی روبین توتال پس از درمان نیز 42% محاسبه شده است. نتیجه گیری: برتری نسبت بیلی روبین به آلبومین در مقایسه با بیلی روبین توتال در ارتباط با پاسخ درمانی به اثبات نرسید ولی می تواند یک معیار با ارزش در کنار بیلی روبین توتال باشد.

The evaluation of relation between bilirubin to albumin ratio in first day of admitted icteric neonates and response to treatment

English Abstract

Background and Objectives: Neonatal jaundice is one of the most common problems in the neonatal period. Bilirubin encephalopathy (kernicterus) is the gravest outcome that may result from severe indirect unbound hyperbilirubinemia. This study is designed to measure the bilirubin to albumin ratio and assess the relation between this and response to treatment. Materials and Methods: 100 healthy term neonates that admitted with icter in booali hospital were selected. After physical examination and achieving the agreement of parents, blood samples were drawn for serum bilirubin and albumin .After collecting all information the data analysis was performed with the SPSS 19 software and described with t test and pearson product-moment correlation coefficient(r). Results: the mean of albumin in groups was not differing significantly (4,6±0.1). The mean of bilirubin to albumin ratio in group with ≤48h phototherapy was 3.6 and 4.1 in the other group. The relation between this ratio and total bilirubin after treatment was 42% (pv=0.001). This relation also found for total bilirubin before treatment and after treatment. Conclusion: there was not significant different between the relation of bilirubin to albumin ratio and total bilirubin in response to treatment. But it can be useful as an additive criterion in companionship with total serum bilirubin.
