تاثير مصاحبه انگيزشی بر پای‌بندی به درمان در افراد مبتلا به پرفشاری خون

Persian Abstract

اهداف: پرفشاری خون يکی از شايع‌ترين بيماری‌های مزمن در جهان است و بسياری از بيماران مبتلا به فشار خون به‌دليل عدم پای‌بندی به درمان از مشکلات فشار خون رنج می‌برند. هدف مطالعه حاضر، بررسی تاثير مصاحبه انگيزشی بر پای‌بندی به درمان بيماران مبتلا به فشار خون و مقايسه آن با مراقبت‌های استاندارد بود. مواد و روش‌ها: اين کارآزمايی بالينی تصادفی‌ شده از بهمن 1392 تا آبان 1393 انجام شد. در مجموع 140 فرد مبتلا به فشار خون، از دو مرکز بهداشتی درمانی واقع در شهر گرگان با توجه به معيارهای ورود به مطالعه به‌روش نمونه‌گيری خوشه‌ای چندمرحله‌ای انتخاب شدند و به‌طور تصادفی در دو گروه مصاحبه انگيزشی (70 نفر) و گروه کنترل (70 نفر) قرار گرفتند. ابزار جمع‌آوری اطلاعات شامل پرسش‌نامه جمعيت‌شناختی (اطلاعات فردی، شاخص‌های آزمايشگاهی و علايم بالينی)، پرسش‌نامه عمومی خودکارآمدی و پرسش‌نامه پای‌بندی به درمان در بيماران مبتلا به فشار خون بود. اطلاعات از طريق نرم‌افزار SPSS 18 و با استفاده از آزمون‌های آماری استنباطی T وابسته و T مستقل مورد تجزيه و تحليل قرار گرفتند. يافته‌ها: اجرای مصاحبه انگيزشی باعث افزايش نمره کل و ميانگين نمره تمام ابعاد پرسش‌نامه پای‌بندی به درمان (به‌جز کنترل وزن و کاهش استرس) شد (05/0 p< ). همچنين بعد از 6 ماه فشار خون سيستولی و دياستولی در گروه مصاحبه انگيزشی به‌طور معنی‌داری کاهش يافته بود (05/0 p< ). نتيجه‌گيری: اجرای مصاحبه انگيزشی به‌منظور بهبود و ارتقای تعهد به اجرای مداخلات درمانی و کنترل فشار خون در بيماران مبتلا به فشار خون مناسب و اثربخش است.

Effect of Motivational Interviewing on Adherence to Treatment in Patients with Hypertension

English Abstract

Aims: Hypertension is one of the most common chronic diseases all around the world and majority of hypertensive patients suffer from hypertension problems due to lack of treatment adherence. The current study aimed to investigate the effect of motivational interviewing on adherence to treatment in patients with hypertension and its comparison with standard cares. Materials & Methods: This randomized clinical trial was done during February to October 2014. In total, 140 of hypertensive patients were selected using multi-stage cluster sampling method from two urban health centers located in Gorgan City, Iran, with regards to the inclusion criteria and allocated randomly into two groups; motivational interviewing (70 participants) and control (70 participants). Data collecting instruments were demographic questionnaire (personal information, laboratory indices and clinical symptoms), general selfefficacy scale and treatment adherence questionnaire of patients with hypertension. Data were analyzed in SPSS 18 software using paired T and Independent T tests. Findings: Motivational interviewing increased total score and the mean score of all dimensions of the adherence questionnaire (except for weight control and stress reduction) (p<0.05). After 6 month, systolic blood pressure and diastolic blood pressure also decreased significantly in the motivational interviewing group (p<0.05). Conclusion: Motivational interviewing is likely beneficial to improve and promote medication adherence and blood pressure control in patients with hypertension.
