بررسی تغییرات هویت جنسی در بیماران مبتلا به سرطان پستان و عوامل موثر بر آن

Persian Abstract

مقدمه و هدف: سرطان پستان شایعترین بدخیمی در میان زنان در جهان بوده و تعداد موارد ابتلا رو به افزایش است. در ایران نیز شایعترین بدخیمی زنان می باشد،به طوریکه 76% از سرطان های آنها را تشکیل میدهد. همچنین پنجمین علت مرگ ناشی از سرطان در میان زنان بوده که باعث حدود 1200 مرگ در هر سال می شود.ماستکتومی می تواند موجب احساس نقص و ناتوانی، اختلال در تصویر ذهنی از بدن،کاهش جذابیت و عملکرد جنسی شده و زمینه را برای ایجاد اختلال جنسی فراهم کند.هدف از این مطالعه بررسی تغییرات هویت جنسی در بیماران مبتلا به سرطان پستان و عوامل موثر بر آن می باشد. مواد وروش ها: در مطالعه کیفی از روش های تصادفی جهت نمونه گیری استفاده نمیشود. روش گرد آوری اطلاعات در این مطالعه، مصاحبه از طریق مصاحبه ی نیمه ساختاری بود. در مصاحبه ای که انجام شده تعدادی سوال باز و کلی تهیه شد.تحلیل داده ها در این مطالعه از طریق کد گذاری انجام شد که شامل دو مرحله کد گذاری باز و کد گذاری محوری بود. یافته ها: حاصل مطالعه شامل 3 طبقه بود. طبقه اول شامل انتظارات نقشی بود. طبقه دوم هویت جنسی گم شده(بحران هویتی)و زیر طبقات آن تغییرات بدنی درک شده ،رفتارهای جنسی تغییر یافته و عوامل تشدید کننده بحران هویتی که این زیر طبقه دارای 4 زیر مجموعه ی رفتار ها و انتظار های شریک جنسی ،احساسات درک شده منفی ، ناتوانی در براورد انتظارات نقشی ،باورهای نادرست جامعه در مورد بیماری بود.طبقه سوم بازسازی مجدد هویت جنسی که زیر طبقات تغییر و تعدیل انتظارات نقشی ، حمایت و تلاش برای بازسازی جسمی بعد ماستکتومی بود. نتیجه گیری: به دنبال سرطان سینه وماستکتومی تصویر ذهنی زنان از خود و هویت زنانگی آنها مختل میشود که در همه جوانب زندگی آنها تاثیر میگذارد.دچار مشکلات خانوادگی میشوند که برای بازسازی آن نیازمند حمایت،انجام اقدامات توانبخشی،مشاوره و پیگیری است.

The sexual identity changes in patients with breast cancer and factors affecting it

English Abstract

Introduction: Breast cancer is the most common malignancy among women in the world and the number of cases is increasing. Iran is the most common malignancy in women, So that they constitute 76% of cancers. The fifth-leading cause of cancer death among women and causes about 1,200 deaths per year.mastectomy could feel mutilation, impaired mental image of the body, reducing the attractiveness and sexual function and grounds for interference provide sex. The aim of this study was to evaluate gender identity in patients with breast cancer and the factors affecting it. Methods: In a qualitative study of random method of sampling used. Data collection techniques in this study , interviews were semi-structured interviews. In an interview conducted a number of questions open were prepared. The data in this study through the coding was done in two stages open coding and axial coding. Results: The results of the study consists of 3 floors. The first floor includes the expectations of the role. Second floor, lost sexual identity (identity crisis) and the following classes perceived physical changes, altered sexual behavior and identity crisis aggravating factors that the floor has 4 sub-categories of sexual behavior and partner expectation, perceived negative emotions and ability to role expectations, misconceptions about the disease. Third floor reconstruction gender role expectations that the modified classes, support and physical reconstruction efforts after the mastectomy. Conclusion: Breast cancer and mastectomy for women's self-image and identity of femininity in all aspects of life disturbed that affect them. Marital problems that need support for its reconstruction, rehabilitation programs, counseling and follow-up.
