بررسی تأثیر تجویز منیزیوم بعد از عمل در کاهش آریتمیهاي بعد از جراحی باي پس قلبی

Persian Abstract

زمینه و هدف: آریتمی هاي قلبی بعد از عمل جراحی باي پس قلبی یکی از شایعترین عوارض عمل جراحی قلب میباشد؛ که خود میتواند عوارض دیگري ایجاد کند. تأثیر سطح سرمی منیزیوم، درمان هیپومنیزیومی و ت جویز منیزیوم پروفیلاکسی در ایجاد و پیشگیري این آریتمی ها موردبحث است و اختلاف نظرهاي بسیاري وجود دارد. هدف از این مطالعه بررسی اثر تجویز منیزیوم در کاهش آریتمی هاي قلبی بعد از جراحی باي پس قلبی است. روش کار: طی این مطالعه کار آزمایی بالینی 140 بیمار کاندید جراحی باي پس قلبی وارد مطالعه شدند. بر اساس سطح خونی منیزیوم اولیه به دو گروه هیپومنیزیوم و نرمومنیزیوم تقسیم شدند.گروه هیپومنیزیوم با دریافت منیزیوم به سطح خونی نرمال رسید. هر دو گروه با سطح خونی نرمال جراحی شدند. بعد از جراحی هر دو گروه بهصورت تصادفی به دو زیرگروه تقسیم شده و به یک زیرگروه 2گرم سولفات منیزیوم تزریق و زیرگروه بعد پلاسبو دریافت کرد. زیرگروهها به مدت 3 روز از نظر رخداد آریتمی ها در بخش مراقبت هاي ویژه جراحی قلب باز تحت نظر بودند. سپس داده هاي حاصل با آزمونهاي آماري مورد تحلیل قرار گرفتند. یافته ها: نتایج حاصل از این مطالعه حاکی از آن بود که ایجاد آریتمی در هیچ یک از زیرگروه ها با هم دیگر تفاوت همچنین ایجاد آریتمی ها با سطح خونی منیزیوم در هیچ یک از روزها ارتباط معناداري .(p>0/ معناداري نداشت ( 05 .(p>0/ نداشت ( 05 نتیجه گیري: سطح خونی منیزیوم و تجویز منیزیوم در کاهش آریتمی هاي بعد از جراحی باي پس قلبی نقشی ندارد. بیشترین میزان بروز آریتمی در گروه هیپومنیزیومی و بدون دریافت پروفیلاکسی در روز سوم بعد از عمل بود که این تفاوت از لحاظ آماري معنی دار نبود.

Effect of Magnesium on Reducing Arrhythmias in Patients after CABG Surgery

English Abstract

Background & objectives: Cardiac arrhythmia after CABG surgery is a common complication which results in other side effects. Therapeutic effect of prophylactic magnesium administration is controversial and there are many different ideas in this case. The aim of this study was to evaluate the therapeutic effects of magnesium in reducing cardiac arrhythmia after CABG. Methods: The clinical trial enrolled 140 patients undergoing CABG. Based on the initial blood levels of magnesium, patients were divided into two groups, one group with low serum magnesium and the other group with normal one. The low serum magnesium group treated with magnesium preoperatively and had normal serum magnesium level before operation. Postoperatively, both groups were randomly divided into two sub-groups, one receiving 2 gr. of magnesium sulfate and the other group received placebo. Both groups monitored for occurrence of arrhythmia in the ICU – OH for 3 days. Data were analyzed with statistical methods. P value of <0.05 was considered statistically significant. Results: The results showed that the occurrence of arrhythmia in any of the sub-groups was not significantly different from each other (p> 0.05). There was no significant relationship between blood levels of magnesium and arrhythmia at different days (p> 0.05). Conclusions: Blood level of magnesium and prophylactic magnesium administration have not effect on reducing arrhythmia after CABG surgery. The highest incidence of arrhythmias happened in the hypomagnesium group without prophylaxis on the third day after surgery, but this difference was not statistically significant.
