تاثیر ملاقات خانواده بر میزان اضطراب بیماران مبتلا به آنژین قلبی بستری در بخش مراقبت ویژه قلبی

Persian Abstract

زمینه و هدف: در ایران حضور خانواده بیمار در بخش مراقبت ویژه ممنوع بوده و بیمار به تنهایی در این بخش بستری میشود. یکی از دلایل این مساله تصور بر افزایش اضطراب بیمار در نتیجه حضور خانواده بر بالین وی می باشد. این مطالعه با هدف تعیین تاثیر ملاقات خانواده بر میزان اضطراب بیماران مبتلا به آنژین قلبی بستری در بخش مراقبت ویژه قلبی بیمارستان شهید رجایی گچساران انجام گرفت. روش کار: این مطالعه یک تحقیق نیمه تجربی قبل و بعد با گروه کنترل بود. جامعه پژوهش را کلیه بیماران مبتلا به آنژین قلبی بستری در بخش مراقبت ویژه قلبی بیمارستان شهید رجایی گچساران که هوشیار بوده و تاریخچه بیماری های روانی یا مصرف داروهای روانگردان را نداشتند تشکیل می دادند. تعداد 68 بیمار به صورت در دسترس از بین جامعه پژوهش انتخاب شدند. روش ملاقات برنامه ریزی شده به صورت دو بار در روز و هر بار به مدت 2 ساعت روی 30 نفر از بیماران حائز شرایط مطالعه اعمال و اضطراب آنها قبل و در پایان مداخله توسط پرسشنامه اضطراب اشپیلبرگر سنجیده شد. در گروه کنترل، روش ملاقات ممنوع که روتین بخش مراقبت ویژه قلبی بیمارستان شهید رجایی گچساران است اعمال و اضطراب بیماران 12 و 72 ساعت پس از بستری اندازه گیری شد. در نهایت میزان اضطراب بیماران دو گروه مداخله و کنترل با استفاده از آزمونهای آماری تی مستقل و تی زوج مقایسه گردید. یافته ها: نتایج نشان داد که در هر دو گروه، میزان اضطراب بیمار سه روز پس از بستری شدن کاهش یافته بود که این کاهش در گروه ملاقات برنامه ریزی شده در مقایسه با گروه ملاقات ممنوع از نظر آماری معنی دار بود (P=0.004). نتیجه گیری: روش ملاقات برنامه ریزی شده شیوه مناسبی برای کاهش ا ضطراب بیماران مبتلا به آنژین قلبی بستری در بخش مراقبت ویژه قلبی است، لذا می تواند جایگزین روش ملاقات ممنوع در بخشهای ویژه گردد.

The Impact of Family Visits on the Level of Anxiety in Patients with Angina Pectoris Hospitalized in Intensive Care Units

English Abstract

Background & objectives: The presence of patient’s family in critical care units in Iran is banned and the patient is alone in these wards. One reason for this is thought to increase patient’s anxiety is a result of his/her family's presence in bedside. This study aimed to determine the impact of family visits on the level of anxiety in patients with angina pectoris hospitalized in cardiac intensive care unit, Iran Methods: This study was an after and before quasi-experimental research with a control group. The study population included all patients with angina pectoris admitted to the CCU of martyr Rajai Hospital in Gachsaran and had no history of psychiatric disorders or psychotropic drugs abuse. A total of 60 patients were selected by convenience sampling method from the study population. Scheduled visits twice a day, each time 1 hour used in 30 patients eligible for the study and their anxiety before and at the end of the intervention was measured by the Spielberger Questionnaire. In the control group, routine prohibited visits were followed and the level of patients’ anxiety was measured 12 and 72 hours after admission. Finally, the level of anxiety in two intervention and control groups was compared using independent and paired t-test. Results: The results showed that 3 days after hospitalization anxiety was reduced in both groups but this reduction is statistically significant in the intervention group (p=0.004). Conclusion: Scheduled visit method is a good way to reduce anxiety in patients with Angina pectoris hospitalized in cardiac intensive care unit, so it may be replaced for banned visiting in intensive care units.
