خلاصه فارسی

اصطلاح MIH برای توصیف نمای کلینیکی مینای هیپومینرالیزه با منشا سیستمیک که مولر اول دائمی واغلب اینسایزرها را درگیر می کند به کار می رود.دندان های هیپومینرالیزه اپسیته های مشخصی در 3/1انسیزالی یا اکلوزالی داشته و مینای آنها دچار شکستگی می شود.اپن اپسیته ها نقص هایی هستند که بدون کاهش در ضخامت، ترنسلوسنسی مینا را تغییر داده ورنگ آن را به سفید- کرمی یا زرد –قهوه ایی تبدیل می کنند.هر چند اتیولوژی دقیق آن ناشناخته است ولی به نظر می رسد شرایط سیستمیک یا محیطی در طول سه سال اول زندگی کودک در ایجاد این هیپومینرالیزاسیون نقش داشته باشد.درمان کلینیکی MIH بنا به دلائل زیر چالشهایی را برای دندانپزشک ایجاد می کند: 1-مستعد بودن این دندانها به ایجاد پوسیدگی وپیشرفت سریع آن 2-همکاری کمتر کودکان برای درمان 3-مشکلاتی در دستیابی به بی حسی موفق 4-شکستگی مکرر مارجین های ترمیم با توجه به افزایش شیوع MIH ومشکلاتی که در این زمینه برای دندانپزشکان مطرح است،تشخیص سریع MIH در کودکان شرایطی را فراهم می کند که اقدامات پیشگیری کننده هر چه سریع تر انجام شود.هدف از این مطالعه بررسی علل ،تشخیص و ارائه رویکردی درجهت درمان MIH می باشد.

The term molar incisor hypomineralization (MIH) was introduced to describe the clinical appearance of enamel hypomineralization of systemic origin affecting one or more permanent first molars (PFMs) that are associated frequently with affected incisors. A tooth was considered to be hypomineralized if it had a demarcated opacity in the incisal or cuspal one third of the crown and broken-down hypomineralized enamel. Demarcated opacities are defects of altered enamel translucency;the defective enamel is white-cream or yellow-brown in color, of normal thickness with a smooth surface. Although the etiology is unclear and may,in fact, be multifactorial it should be emphasized that the systemic conditions or environmental insults during the child’s first 3 years have been implicated.MIH’s clinical management is challenging due to: 1. the sensitivity and rapid development of dental caries in affected PFMs; 2. the limited cooperation of a young child; 3. difficulty in achieving anesthesia; and 4. the repeated marginal breakdown of restorations. The prevalence of MIH appears to be increasing, and managing affected children is now a common problem for dentists. The early identification of such children will allow monitoring of their PFMs so that remineralization and preventive measures can be instituted as soon as affected surfaces are accessible. The aim of this study was to determine the etiology ,diagnosis and treatment of MIH and to present a sequential approach to management.
