بررسی ویژگی های روان سنجی و تعیین ابعاد مقیاس بررسی علائم ادمونتون (ESAS) در بیماران مبتلا به سرطان بیمارستان امام خمینی (ره) اردبیل، سال 1395

Persian Abstract

زمینه و هدف: سیستم بررسی علائم ادمونتون (ESAS) برای بررسی علائم ذهنی و عینی بیماران مبتلا به سرطان طراحی شده است. هدف از این مطالعه ترجمه و تعیین خصوصیات روانسنجی ابزار ESAS در جامعه ایران شده ات. مواد و روش ها: مطالعه حاضر روائی و پایایی ابزار ESAS را بر 246 بیمار مبتلا به سرطان در بیمارستان امام خمینی (ره) اردبیل مورد بررسی قرار داد. پس از ترجمه ابزار به زبان فارسی، روایی محتوایی و صوری ابزار، ثبات درونی، آزمون-آزمون مجدد جهت تعیین خصوصیات روانسنجی ESAS مورد بررسی قرار گرفت. علاوه براین، روایی سازه با استفاده از آنالیز عاملی تاییدی جهت تعیین ساختار عاملی این ابزار در دو مدل یک بعدی و سه بعدی مورد بررسی قرار گرفت. نتایج: با توجه به شاخص های نکویی برازش از جمله شاخص برازش مقایسه ای، شاخص برازش افزایشی و شاخص برازش نرمال شده ساختار عاملی ESAS در قالب یک بعد و نه آیتم مورد تایید قرار گرفت. بعلت آنکه میانگین واریانس استخراج شده برای هر ساختار از مجذور همبستگیبین عامل ها پایین تر بوده است، لذا ساختار سه بعدی ابزار ESAS مورد تایید قرار نگرفت. روائی روائی افتراقی با توجه به وجود اختلاف قابل توجه بین دو گروه بیماران بدحال و بیماران با وضعیت عمومی بهتر تأیید گردید.ضریب آلفا کرونباخ برای کل ابزار 0.88 بدست آمد و و همبستگی بین آزمون و آزمون مجدد با وقفه 6-4 ساعت، 0/86 (r = 0.86 P < 0001) بدست آمد. نتیجه گیری: نتایج نشان داد که نسخه فارسی ESAS ساختار عاملی مشابه با نسخه اصلی خود دارد و این ابزار قابلیت اجرا بمنظور بررسی علائم عینی و ذهنی بیماران مبتلا به سرطان در جامعه ایران را دارا می باشد.

Psychometric Properties and Factor Structure of Persian Version of Edmonton Symptom Assessment Scale in Cancer Patients of Imam Khomeini hospital, Ardabil, Iran

English Abstract

Background & objectives: Edmonton Symptom Assessment Scale (ESAS) was developed to assess objective and subjective symptoms in patients with cancer in all stages of their disease. The aim of the study was to translate and determine the psychometric properties of ESAS in an Iranian population. Methods: The current study was carried out to determine reliability and validity of ESAS using 246 patients with cancer in Imam Khomeini Hospital, Ardabil, Iran. After translating the instrument to Persian, content and face validity, discriminant validity, internal consistency, and test-retest were done to determine psychometric properties of ESAS. Furthermore, the construct validity was determined using confi rmatory factor analysis to evaluate factor structure of the tool in two models: single factor and three factor. Results: With regard to goodness of fi t indices including comparative fi t index, incremental fi t index, and normed fi t index, factor structure of ESAS was confi rmed with one factor and nine items. Because average variance extracted of each construct was less than the square of correlation coeffi cients between the respective constructs, three-factor model was not confirmed. Discriminant validity was confi rmed by finding significant differences between the two groups, patients with good general and critically ill conditions. Cronbach’s alpha for the overall ESAS was 0.88, and correlation between test-retest with 4–6 h interval was 0.86 (r = 0.86 P < 0001). Conclusions: This study showed that Persian version of ESAS with same factor structure mentioned in the original version is an applicable tool for assessing objective and subjective symptoms in Iranian patients with cancer.
