بررسی ارتباط بین سطح لاکتات سرم و Vo2/Vco2 در بیماران تحت CABG در بیمارستان امام خمینی اردبیل در سال 1396

سابقه و هدف :Vo2/Vco2 و لاکتات خون شاخص های مختلفی از کفایت دریافت اکسیژن می باشند. هدف از این مطالعه ، بررسی ارتباط بین لاکتات خون و Vo2/Vco2 برای پیش بینی نتایج در عمل جراحی CABG زیر پمپ می باشد. مواد و روش ها : این مطالعه مقطعی تحلیلی و تصادفی شامل 30 بیمار تحت CABG است. اطلاعات دموگرافیک بیماران شامل سن و جنس جمع آوری شد. مانیتورینگ استاندارد شامل ؛ الکتروکاردیوگرام ، فشار شریانی invasive ،پالس اکسی متری ، end-tidal (تغییرات ) کربن دی اکسید ، درجه حرارت و out-put ادراری انجام شد. گرفت بای پس شریان کرونری از طریق پمپ CPB در زمان گرفتینگ انجام شد. لاکتات خون و Vo2/Vco2 در 3 گروه از بیماران اندازه گیری شدند (تعداد گرفت ، کسر EF و کراتینین سرم).اثر لاکتات خون و Vo2/Vco2 اندازه گیری شده بر روی مدت زمان بستری در ICU نیز بررسی شد. یافته ها :اکثر بیماران مذکر بودند (86.7%).میانگین سنی بیماران 58.10 سال بوده است.میزان لاکتات خون و Vo2/Vco2 بعد از عمل جراحی افزایش یافتند ولی تفاوت آماری واضحی در دو گروه تعداد گرفت و کراتینین سرم نداشتند. ولی از لحاظ آماری در گروه کسر EF معنادار بودند.بین لاکتات خون و Vo2/Vco2 همبستگی وجود داشت. نتیجه گیری : پایش لاکتات خون وVo2/Vco2 همراه با هم در بیماران تحت CABG با پمپ دارای حساسیت و خاصیت بالا در پیش بینی نتایج است.کما اینکه این دو متغیر، براساس نتایج مطالعه ما با یکدیگر همبستگی داشته و تغییرات سطوح آن با یکدیگر مرتبط بوده است. پیشنهاد می شود مطالعات آینده در ابعاد گسترده تر با حجم نمونه بیشتر ، طیف وسیع تر بیماران و در عمل های جراحی متفاوت پیگیری شود.

Correlation of Blood lactate and VO2/VCO2 in patients undergoing CABG in Imam Hospital of Ardebil

English Abstract

Backgorund&objective: Vo2/Vco2 and blood lactate are different indices of adequacy of oxygen delivery to oxygen needs .The objective of this study is to evaluate the correlation of blood lactate and Vo2/Vco2 to predict outcomes in on-pump coronary artery bypass grafting. Methods: This is an analytical cross-sectional and randomized study including 30 patients undergoing CABG. Demographic data of patients including age and sex were collected. Standard monitoring included electrocardiogram, invasive arterial pressure, pulse oximetry, end-tidal carbon deoxidation, temperature and urine out-put was done.The coronary artery bypass was carried out through a CPB pump at the time of grafting. Blood lactate and Vo2/Vco2 were measured in 3 groups of patients (number of grafts, ejection fraction and serum creatinine). The measured blood lactate and Vo2/Vco2 are compared to assess the outcomes in terms of intensive care unit stay. Results: The majority of patients were male (86.7%). The mean age of patients was 58.10 years. The blood lactate and Vo2/Vco2 values increased post operatively but there was no statistically significant difference in two groups— number of grafts and Serum Cr.It was statistically significant in ejection/fraction group.There was a significant correlation between Blood lactate and vo2/Vco2. Conclusion: Monitoring of blood lactate and Vo2/Vco2 together in CABG patients has high sensitivity and potentials in predicting the results,As these two variables correlate in our study and the changes in levels have been correlated with each other. We suggest future studies to be conducted on a larger sample size, a wider range of patients, and different surgical procedures.
