بررسی یافته های آنژیوگرافی کرونری بیماران مراجعه کننده با سندرم ولنز به بیمارستان امام خمینی(ره) اردبیل

Persian Abstract

سابقه و هدف : سندرم ولنز(Wellens' syndrome) به اختلالات الکتروکاردیوگرافی خاص در موج T (موج Tمعکوس و بای فازیک )در لید های قدامی اشاره دارد ،که با تنگی پروگزیمال شریان کرونری نزولی قدامی چپ (LAD) همراه است.هدف از این مطالعه بررسی یافته های آنژیوگرافی کرونری بیماران مراجعه کننده با سندرم ولنز به بیمارستان امام خمینی(ره) اردبیل بود. مواد و روش ها: در این مطالعه بیمارانی که در تیر تا شهریور 96 در بیمارستان امام خمینی (ره) اردبیل تحت آنژیوگرافی قرار گرفته و دارای سابقه ای از آنژین صدری بوده و ویزگی های سندرم ولنز را داشتند مورد بررسی قرار گرفتند. اطلاعات بیماران از نظرسن، جنس، ریسک فاکتورهای بیماری عروق کرونری و نتایج آنژیوگرافی تحت بررسی قرار گرفت. یافته ها: در این مطالعه میانگین سنی کلی بیماران 10/67±58/7 سال بود.در مطالعه ما بر خلاف انتظار شیوع سندرم ولنز در زنان بیشتر از مردان بود. %32/5 بیماران دارای زمینه دیابت و %72/5 دارای زمینه فشار خون بالا بودند.بیشترین تغییرات در نوار قلب به صورت موج T معکوس در لید های V1-V2 با فراوانی%30 بود.بررسی آنژیوگرافی عروق کرونر نشان داد که شریان نزولی قدامی چپ با فراوانی %75 بیشترین رگ درگیر و بیشترین محل تنگی در این رگ قسمت یک سوم میانی بود.%35 بیماران در شریان چرخشی چپ و%27/5 در شریان کرونری راست درگیری داشتند.در مجموع یافته های آنژیوگرافی نشان داد بیشترین میزان درگیری مربوط به درگیری دو رگ با %25 بود. نتیجه گیری: براساس یافته های این مطالعه،شیوع سندرم ولنز در زنان بیشتر از مردان بود.بیشترین تغییرات موج در لیدهای مجاور V1-V2 بود.شایعترین رگ کرونری درگیر مثل مطالعات دیگر،شریان نزولی قدامی چپ و شایعترین محل تنگی در شریان فوق،برخلاف مطالعات دیگر،یک سوم میانی بود.

Angiocardiographic findings in patients with Wellens' syndrome who admite to Emem Khomeini Hospital

English Abstract

Background and Objective:Wellens’ syndrome is a pattern of electrocardiographic Twave changes(biphasicT-wave inversion) in pericardial leads associated with critical stenosis of proximal left anterior descending artery (LAD). The aim of this study was to determine the angiocardiographic findings in patients with Wellens' syndrome who admited to Emam Khomeini Hospital in Ardabil. Methods: patients with history of recent chest pain who electrocardiographic findings were compatible with Wellens' syndrome and were underwent angiography in Ardabil Emam Khomeini Hospital between July and September in 2017 were studied. All patients' data as age,gender, risk factors for coronary artery disease and the results of angiography were collected and were analyzed by SPSSv22.Results: in this study, the mean age of patients was 58.7 ± 10.67.The number of female patients was higher than male patients (32.5% male and 67.5% female). 32.5% of patients had diabetes history and 72.5% had a history of blood pressure.Most of the Twave changes on ECG was inverted T- wave in V1-V2 leads(30%).Angiographic findings showed that 75% of the subjects had involvement in the LAD and most of the stenosis was in the mid part of the artery. In 35% of the patients, the LCX vessel had stenosis. Also, 27.5% of the subjects had involvement in the RCA.Conclusion:Result of this study showed that,unexpectedly prevalence of Wellens' syndrome in women was more than men.Most of the Twave changes on ECG was inverted T- wave in V1- V2 leads.Like other studies,the most common artery that had stenosis was LAD and the most common site of the lesion was Mid part of LAD.
