ترجمه و روانسنجی مقیاس بررسی علائم ادمونتون

Persian Abstract

زمینه و هدف: مقیاس بررسی علایم ادمونتون (ESAS) ابزاری است که به بررسی مشکلات ذهنی و عینی بیماران مبتلا به سرطان در مراحل مختلف بیماری می­پردازد. این پژوهش با هدف ترجمه و تعیین ویژگی­های روان­سنجی «مقیاس بررسی علایم ادمونتون» در جامعه ایران انجام شد. روش کار: پژوهش حاضر به روان­سنجی مقیاس بررسی علایم ادمونتون ESAS در 246 بیمار مبتلا به سرطان در بیمارستان امام خمینی (ره) شهر اردبیل در سال 1395 پرداخت که این تعداد نمونه با توجه به اینکه برای انجام روانسنجی 3 تا 20 نمونه به ازای هر آیتم و برای آزمون- ­آزمون مجدد حداقل 50 نمونه ضرورت داشت، انتخاب شد. پس از ترجمه ابزار به زبان فارسی، به منظور روان سنجی ابزار، از بررسی روایی محتوایی و صوری، روایی افتراقی، روایی سازه (تحلیل عاملی تأییدی)، پایایی همسانی درونی و پایایی ثبات استفاده گردید. جهت بررسی سازه عاملی ابزار، از تحلیل عاملی تأییدی با استفاده از نرم افزار لیزرل نسخه 8/8 و برای سایر موارد از SPSS-15 استفاده شد. یافته ها: با توجه به مناسب بودن شاخص­های نیکویی برازش از جمله شاخص برازش تطبیقی، شاخص مناسب افزایشی، شاخص تناسب هنجار شده ساختار عاملی یک بعدی، ابزار با تمام 9 آیتم آن مورد تأیید قرار گرفت. با توجه به اینکه نمره ESAS بطور معنی­داری در گروه بیماران بدحال بالاتر از بیماران با حال عمومی خوب بدست آمد، روایی افتراقی ابزار نیز تأیید گردید. ضریب آلفا برای کل ابزار 0/88 و همبستگی بین دو بار اجرای آزمون با فاصله 4 الی 6 ساعت، 0/86 به دست آمد (0/0001>p، 0/86=r). نتیجه­ گیری: پژوهش حاضر نشان داد که نسخه فارسی ESAS از قابلیت کاربرد برای اندازه­گیری علائم آزاردهنده در بیماران مبتلا به سرطان برخوردار است. پژوهش حاضر می­تواند گامی برای کاربرد وسیع این مقیاس در تمام مراکز درمانی، توانبخشی و مراقبت تسکینی مرتبط با بیماران مبتلا به سرطان باشد.

Translation and Psychometrics of Edmonton Symptom Assessment Scale

English Abstract

Background and aim: Edmonton symptom assessment scale (ESAS) has been developed to assess objective and subjective symptoms in patients with cancer in all stages of their disease. The aim of the study was to translate and determine the psychometric properties of ESAS in Iranian population. Methods: The current study was carried out to determine the reliability and validity of ESAS using 246 patients with cancer in Imam Khomeini hospital, Ardabil, Iran in 2016. After translating the instrument into Persian, content and face validity, discriminant validity, construct validity (confirmatory factor analysis), internal consistency and test-retest were performed to determine the psychometric properties of ESAS. To determine construct validity of the scale, confirmatory factor analysis was used by LISREL software version 8.8, and the other analyses were calculated by using SPSS software version 15. Results: Regard to good fitness indices including CFI, IFI, NFI, factor structure of ESAS with one factor were confirmed by the nine items. Discriminant validity was confirmed by finding significant differences between two groups, patients with good and ill status. Cronbach’s alpha for the overall ESAS was 0.88, and correlation between test-retest with 4-6h interval was 0.86 (r=0.86, p<0.0001). Conclusion: This study showed that Persian version of ESAS is an applicable tool for assessing symptoms distress in patients with cancer in Iranian society. This study might be the first step for utilizing this scale in any medical, rehabilitative and palliative care centers related to patients with cancer.
