Persian Abstract

زمينه و هدف: استفاده از فشار با دست برروي ارتفاع رحم (fundal pressur) در مرحله دوم زايمان يك امر معمول و متداول در بسياري از كشورهاي دنياست كه توسط پزشكان و ماماها انجام مي شود و به لحاظ جوانب قانوني و اخلاقي در پروندۀ بيماران ثبت نمي شود. اين مانور كه به نام kisteller نيز به كار برده مي شود (نام ابداع كنندۀ آن) باعث عوارض مادري و جنيني مي شود كه در مطالعات بسياري به آنها اشاره شده است. مقاله مروري فوق با هدف بررسي انديكاسيون ها و كنترا انديكاسيون ها، عوارض مادري و جنيني اين مانور و هچنين پيشنهادها و راهكارهايي براي استفاده مناسب از اين مانور تدوين شده است. اين مطالعه مروري با استفاده از جستجوي كلمات كليدي fundal pressure –second stage، kristeller maneuver در سايتهاي معتبر علمي دنيا انجام شده است.با توجه به مطالعات متعدد، شواهد علمي مبني بر مؤثر بودن و مفيد بودن اين مانور براي كوتاه كردن مرحله دوم زايمان طبيعي وجود ندارد، لذا پيشنهاد مي شود از اين مانور در مرحله دوم زايمان استفاده نشود.

English Abstract

Introduction & Objective: Among the maneuvers that are used in the second stage of labor, uterine fundal pressure is one of the most controversial. The prevalence of its use is unknown. We reviewed the existing literature to assess whether there is justification for the use of fundal pressure in the contemporary management of the second stage of labor. No confirmed benefit of the procedure has been documented and a few adverse events have been reported in association with its use. Alternative management strategies in the second stage of labor exist and should be considered whenever possible. In conclusion, the role of fundal pressure is understudied and remains controversial in the management of the second stage of labor. We believe that caution should be exercised using this maneuver until it is proven to be safe and effective.
