تأثیر مکمل -Lکارنیتین بر تراز ازته، BUN سرم، برخی سنجه‌های هماتولوژیک، مدت بستری و نتیجه عمل جراحی در بیماران سرطان مری

Persian Abstract

مقدمه و هدف: استفاده از مکمل‌ها از جمله –Lکارنیتین، حمایت‌های تغذیه‌ای ایده‌آل و تغذیه زودهنگام بعد از عمل جراحی در سرطان، می‌تواند در پیشگیری از عوارض بیماران مهم باشد. مطالعه حاضر با هدف تعیین تأثیر تغذیه زودهنگام غنی از L -کارنیتین بر عوارض عمل جراحی سرطان مری و طول مدت بستری در بخش‌های بیمارستان انجام گردید. روش کار: در یک کارآزمایی بالینی، 50 نفر از بیماران مبتلا به سرطان مری تحت عمل جراحی در دو گروه مساوی به صورت تصادفی برای بررسی انتخاب شدند. با استفاده از پرسشنامه یادداشت غذائی اطلاعات تغذیه‌ای استخراج گردید. از هر دو گروه قبل از شروع و انتهای مطالعه نمونه خونی پایه و نمونه ادرار 24 ساعته به ترتیب برای اندازه‌گیری فاکتورهای خونی (مثل شمارش سلول‌های خون، هموگلوبین، هماتوکریت و اوره خون) و تراز ازته گرفته شد. سپس به گروه مداخله در اولین روز بعد از عمل جراحی مکمل-L کارنیتین به میزان روزانه 3 گرم در 3 دوز منقسم به همراه رژیم غذائی روتین بیمارستان در هر وعده غذایی و به گروه شاهد رژیم غذائی روتین بیمارستان به مدت ده روز داده شد. داده‌ها با استفاده از آزمون‌های تی مستقل، تی زوجی و کای دو تجزیه و تحلیل شدند. یافته‌ها: نتایج نشان داد که تغییرات میانگین BUN و تراز ازته در دو گروه بعد از مداخله اختلاف معنی‌داری داشت (05/0>p). بیمارانی که تغذیه زودهنگام غنی از-L کارنیتین را شروع کرده بودند، مدت زمان کمتری در بخش‌های بیمارستان سپری کرده و زودتر ترخیص شدند، بطوری‬که این اختلاف با گروه کنترل معنی‌دار بود (05/0>p). نتیجه‌گیری: نتایج این مطالعه نشان می‌دهد که‌ تغذیه زودهنگام غنی ازL کارنیتین احتمالاً بتواند بر طول مدت بستری بیماران مبتلا به سرطان مری بعد از عمل جراحی در بخش‌های بیمارستان تاثیر مثبت بگذارد.

Effects of L-carnitine Supplement on Nitrogen Balance, Serum BUN, Some Hematological Measures, Duration of Hospitalization and Surgical Outcome in Patients with Esophageal Cancer

English Abstract

Background & objectives: The use of supplements such as L-carnitine, optimum nutritional support and early feeding after surgery in cancer patients can be important in the prevention of cancer complications. The aim of this study was to determine the effects of early L-carnitine-rich feeding on complications of esophageal cancer surgery and duration of hospitalization. Methods: In a clinical trial, 50 patients with esophageal cancer under surgery in two equal groups were randomly selected. Nutritional information was obtained using food record questionnaire. Basic blood samples and 24-hour urine samples were taken in order to measure the hematological parameters (like blood cell count, hemoglobin, hematocrit and blood urea) and nitrogen balance, respectively from two groups at the beginning and end of the study. Then, the intervention group received 3 g L-carnitine daily in 3 doses on the first day after the complementary operation, with a routine hospital diet at each meal, and the control group received a routine hospital diet for ten days. Data were analyzed using independent t-test, paired t-test and chi-square test. Results: The results showed that changes in the mean BUN and nitrogen balance were significantly different after intervention between the study groups (p<0.05). Patients with early L-carnitine-rich feeding had less hospitalization time in hospital wards and were discharged earlier (p<0.05). Conclusion: The results of this study showed that early L-carnitine-rich feeding after surgery has probably a positive effect on the duration of hospital stay in patients with esophageal cancer
