مروری بر عوامل مؤثر بر آمادگی پرستاران در زمینه پیاده‌سازی پرونده الکترونیک سلامت

مقدمه: پرونده الکترونیک سلامت (EHR)، جمع‌آوری الکترونیکی اطلاعات تمام طول حیات یک فرد است که با اهداف اصلی ارتقای کیفیت مراقبت، فراهم آوردن ابزاری مؤثر برای ارتباط و اشتراک‌گذاری اطلاعات توسط ارائه‌دهندگان مراقبت بهداشتی ثبت و یا تأیید می‌شود. آمادگی کارکنان مراکز بهداشتی درمانی فاکتور مهمی در اجرای موفقیت‌آمیز پرونده الکترونیک سلامت به شمار می‌آید. در بین ارائه‌دهندگان مراقبت، نقش پرستاران کلیدی و باارزش است. این مطالعه با هدف مروری بر عوامل مؤثر در آمادگی پرستاران در خصوص پیاده‌سازی پرونده الکترونیک سلامت انجام گرفت. روش کار: در این مطالعه مروری روایتی، با استفاده از پایگاه اطلاعاتی داخلی:Magiran ،SID ، Iranmedex و خارجی: CINAHL،ProQuest ،Ovid ،PubMed،Scopus ،Science Direct ، Google Scholar کلیه مقالات چاپ شده در کشور ایران در بازه زمانی 2010 تا 2020 در زمینه آمادگی پرستاران در خصوص پیاده‌سازی پرونده الکترونیک سلامت مورد جستجو قرار گرفت. یافته‌ها: نتیجه جستجو دستیابی به 39 مقاله مرتبط با موضوع بود که نهایتاً 13 مطالعه کاملاً مرتبط وارد پژوهش شد. مطالعات انجام‌شده نشان می‌دهد آمادگی نیروی انسانی عامل مؤثری در پیاده‌سازی موفق پرونده الکترونیک سلامت است که می‌توان عوامل مؤثر را به دو دسته عوامل فردی و سازمانی تقسیم‌بندی نمود. درک و آگاهی پرستاران از ویژگی‌ها و مزایای EHR نقش بسزایی در آمادگی آنها دارد. از دیگر عوامل مؤثر میزان مهارت‌های کامپیوتری پرستاران می‌باشد که جهت کار با پرونده الکترونیک سلامت ضروری است. در این راستا مطالعات متعددی مهارت‌ها، آگاهی و نگرش پرستاران و سایر ارائه‌دهندگان مراقبت سلامت را مورد بررسی قرار داده و مشخص کردند که آموزش ناکافی و غیرمؤثر پرستاران موجب استرس، فرسودگی و احساس بی‌کفایتی در آنها می‌شود از این رو توصیه به برگزاری دوره‌های آموزش عملی و بازآموزی و جلب مشارکت پرستاران نموده‌اند. عامل ایجاد انگیزه از طریق پاداش و تشویق عاملی بود که از دیدگاه پرستاران مهم بود. امنیت و محرمانگی اطلاعات در EHR و مشارکت افراد در طراحی و پیاده‌سازی EHR دو عاملی هستند که در برخی مطالعات به عنوان عوامل مؤثر و در مطالعات دیگری کم اثر گزارش شده‌اند. نتیجه‌گیری: بر اساس نتایج حاصله، پرستاران به عنوان بزرگترین گروه ارائه‌دهندگان مراقبت بهداشتی نقش مهمی در پیاده‌سازی پرونده الکترونیک سلامت دارند از این رو با شناسایی عوامل مؤثر در آمادگی آنها جهت این امر شامل عوامل فردی می‌توان با مشارکت دادن پرستاران، افزایش درک، آگاهی و مهارت‌های آنها در زمینه EHR، انگیزش و تغییر نگرش در این مورد برنامه‌ریزی نمود همچنین در زمینه عوامل سازمانی، مدیران و سیاست‌گذاران مراقبت سلامت با حمایت‌های خود و فراهم نمودن زیرساخت‌ها و تجهیزات لازم و برگزاری برنامه‌های مناسب می‌توانند نقش تسهیل گری خود را ایفا نمایند.

A review of the factors affecting nurses' readiness to implement electronic health records

Introduction: Electronic Health Record (EHR) is a lifelong electronic collection of information recorded or validated by health care providers with the main objectives of improving the quality of care, providing a useful tool for communication and information sharing. The staff of health centers' readiness is an essential factor in successfully implementing the electronic health record. Among care providers, the role of nurses is vital and valuable. This study aimed to review the factors affecting nurses' readiness to implement electronic health records. Methods: In this narrative review study, using the internal database: Magiran, SID, Iranmedex and foreign: CINAHL, ProQuest, Ovid, PubMed, Scopus, Science Direct, Google Scholar All articles published in Iran during 2010 By 2020, nurses' readiness to implement electronic health records were researched. Results: The search results were access to 39 articles related to the subject, and finally, 13 completely related studies were included in the research. Studies show that human resources' readiness is an influential factor in the successful implementation of electronic health records divided into two categories of individual and organizational factors. Nurses' understanding of the features and benefits of EHR plays an essential role in their readiness. Another influential factor is nurses' computer skills, which are essential for working with electronic health records. In this regard, several studies have examined the skills, knowledge, and attitudes of nurses and other health care providers and found that inadequate and ineffective training of nurses causes stress, burnout, and feelings of inadequacy, so it is recommended to hold practical training courses and they have retrained and involved nurses. Motivation through reward and encouragement was a factor that was important from the nurses' point of view. Information security and confidentiality in the EHR and people's participation in the design and implementation of the EHR are two factors that have been reported as influential factors in some studies and ineffective in others. Conclusion: Based on the results, nurses, as the largest healthcare providers, have an essential role in implementing electronic health records. Therefore, identifying the factors affecting their readiness for this, including individual factors, can increase nurses' understanding, awareness, and skills in EHR, motivation, and attitude change planning in this regard. Also, in organizational factors, managers and health care policymakers, with their support and providing the necessary infrastructure and equipment and holding appropriate programs, can play a facilitating role.
