طراحی مدل مفهومی سیستم اطلاعاتی ترخیص بیماران در بیمارستانهای دانشگاه علوم پزشکی اردبیل

زمينه وهدف: سیستم اطلاعاتی ترخیص، یکی از زیر سیستم های مهم بیمارستانی به شمار می آید که عملکرد آن در بحث هزینه ها و کسورات بیمارستانی بسیار حائز اهمیت می باشد. لذا باید از هزینه های ناشی از ناکارآمدی سیستم های اطلاعات بیمارستانی که در اثر عدم انطباق این سیستم ها با نیازهای کاربران به وجود می آید، جلوگیری کرد. صحت و به موقع بودن صورت حساب های بیماران، بر عملکرد بیمارستان و هم چنین دررضایت بیماران و بهبود آن موثر خواهد بود. اجرای نادرست فرایند ترخیص می تواند به عنوان یک عامل منفی در مدیریت تخت، منجر به اتلاف منابع و تجهیزات، نارضایتی بیمار و پرسنل، تأثیر منفی بر کیفیت خدمات و ایمنی بیمار، افزایش پرونده های بلاتکلیف، تأخیر در پذیرش بیماران جدید و شلوغی واحد ترخیص در ساعاتی خاص شود. بنابراين پژوهش حاضر با هدف ارائه مدل مفهومي سيستم اطلاعاتي ترخيص بيماران به عنوان گامي مهم جهت توسعه سيستم انجام شد. روش كار: اين مطالعه، یک مطالعه توصیفی- کمی و از نوع کاربردی بود كه در سال 1400-1399، براي تعيين وضعيت فعلي وتعيين داده هاي مورد نياز كاربران سيستم اطلاعاتي ترخيص با طراحي مدل اوليه به منظور ارائه مدل مناسب در بيمارستان هاي آموزشي دانشگاه علوم پزشكي اردبيل انجام شده است .اعتبار مدل نهايي در دو مرحله تكنيك دلفي، نظرات خبرگان و با استفاده از مرور مطالعات مشابه، مشاهده، مصاحبه و پرسشنامه برای کاربران مجموعه حداقل عناصر داده اي و جريان كاري سيستم تعيين گرديد. در مرحله طراحي مدل مفهومي با استفاده از زبان مدلسازي يكپارچه ،نمودارهاي ساختاري و رفتاري سيستم طراحي شدند. یافته ها: نتايج حاصل از مرحله اول پژوهش، تعيين اهداف، جريان كاري، مجموعه حداقل عناصر داده اي و نيازهاي كاربران سيستم اطلاعاتي ترخيص بود. افزايش آگاهي كاربران در كار با سيستم و ثبت اطلاعات صحيح توسط كاربران باليني(خدمات پارا كلينيك، دارو ولوازم) و اداري (هويتي، بيمه اي و استحقاق سنجي بيمار) از مهم ترين اهداف و انتظارات تعيين شده، توسط شركت كنندگان بود. همچنين، مجموعه حداقل داده تعيين شده، در اين پژوهش شامل 128 عنصر اطلاعاتی در هفت دسته اصلی شامل (اطلاعات شناسایی وفردی بیمار، سطوح دسترسی کاربران بالینی، اداری- مالی وهویتی، اطلاعات بیمه ای بیمار، ارائه دهندگان خدمات سلامت، داده های مالی وصورتحساب بیمار وداده های ترخیص بیمار و صدور قبض وخروج بیمار از مرکز) بود. نتيجه حاصل از مرحله دوم پژوهش، تعيين نيازهاي عملياتي، كنشگرها، فرم هاي مورد كاربرد و همچنين طراحي نمودارهاي مورد كاربرد، فعاليت، توالي وكلاس به عنوان مدل مفهومي بود. نتيجه گيري:مدل يكپارچه حاصل از این پژوهش، مي تواند به عنوان ابزاري موثر در طراحي، توسعه ومديريت سيستم اطلاعاتي ترخيص كاربر محور، مورد بهره برداري قرار گيرد و به موفقيت سيستم هاي اطلاعاتي ترخيص كمك نمايد.

Designing a conceptual model of patient discharge information system in Hospitals of Ardabil University of Medical Sciences

Backgraound and Aim: The discharge information system is one of the most important hospital subsystems, the performance of which is very important in discussing hospital costs and deductions. Therefore, the costs of inefficient hospital information systems due to non-compliance with the needs of users should be avoided. Accuracy and timeliness of patients' bills will affect the performance of the hospital as well as patient satisfaction and improvement. Improper implementation of the discharge process can be a negative factor in bed management, leading to waste of resources and equipment, patient and staff dissatisfaction, negative impact on service quality and patient safety, increase in unresolved cases, delays in admitting new patients and crowded discharge unit. Be at specific times. Therefore, the present study was conducted with the aim of presenting a conceptual model of patient discharge information system as an important step for system development. Methods: This study was a descriptive-quantitative and applied study that in 1300-1400, to determine the current situation and determine the data required by users of the discharge information system by designing a prototype to provide a suitable model in the teaching hospitals of Ardabil University of Medical Sciences done.The validity of the final model was determined in two stages of Delphi technique, expert opinions and by reviewing similar studies, interviewing and observing the set of minimum data elements and system workflow. In the conceptual model design stage, the structural and behavioral diagrams of the system were designed in the first stage of the study using the integrated modeling language. Results: The results of the first stage of the research were setting goals, workflow, minimum data elements and the needs of clearance information system users. Increasing users' awareness in working with the system and recording correct information by clinical users (paraclinic services, drugs and supplies) and compulsion (identity, insurance and patient entitlement) were among the most important goals and expectations set by the participants. Also, the minimum data set determined in this study includes 128 information elements in seven main categories including (patient identification and personal information, clinical user access levels, administrative-financial and identity, patient insurance information, health care providers, financial data and invoices). The result of the second phase of the research was to determine the operational needs, actors, forms used and also to design the diagrams used, activity, sequence and class as a conceptual model. Conclusion: The integrated model obtained from this research can be used as an effective tool in the design, development and management of user-centric clearance information systems and contribute to the success of clearance information systems.
