عوامل موثر در ترمیم دندان ها در دندانپزشکی سالمندان : مطالعه مروری نظام مند

مقدمه: با بالا رفتن مراجعه سالمندان به مطب‌های دندانپزشکی، داشتن اطلاعات کافی در برخورد صحیح و طرح درمان مناسب برای این بیماران ضروری است. بنابراین، با توجه به خلأ اطلاعاتی در این زمینه، هدف مطالعه حاضر مرور نظام مند ملاحظات ترمیمی برای سالمندان است. روش کار: در این مطالعه مرور نظام مند پایگاه¬های اطلاعاتی Pubmed، Embase، Web of Science، Scopus، Proquest، موتور جستجوی Google Scholar و پایگاه های ایرانی با کلیدواژه های دندانپزشکی سالمندان و ملاحظات ترمیمی و معادل انگلیسی آنها شامل Geriatric dentistry و Restorative consideration جستجو شدند. معیار ورود به مطالعه، مقالات اصیل و مروری مرتبط با ملاحظات دندانپزشکی ترمیمی برای سالمندان بدون محدودیت زمانی و به زبان انگلیسی یا فارسی بود. به منظور ترکیب نتایج پژوهش¬ها از روش تحلیل محتوای تماتیک استفاده شد. نتایج: تعداد کل مدارک بازیابی شده 4208، تعداد مدارک تکراری 2042 و تعداد مدارک مرور شده 17 بود. بیشتر مطالعات حذف شده به ملاحظات ترمیمی افراد غیر سالمند اشاره کرده و یا مطالعات مرتبط با دندانپزشکی سالمندان که در خصوص نکات ترمیمی بحث نکرده بودند. بیشتر این مطالعات با روش کارآزمایی بالینی، مرور متون، و توصیفی گذشته نگر انجام شده بودند. همچنین، عمده این مطالعات مربوط به کشور ایرلند با چهار مطالعه، آمریکا با سه مطالعه و هنگ کنگ بود. هلند، ایران، استرالیا، فنلاند، عربستان، کلمبیا، و سنگاپور هر کدام یک مطالعه را به خود اختصاص داده بودند. به طور کلی، ملاحظات ترمیمی در سالمندان در سه موضوع ملاحظات مرتبط با خود سالمندان، ملاحظات مرتبط با روش ها و مواد ترمیمی و ملاحظات مرتبط با تجربیات دندانپزشکان جای گرفت. نتیجه گیری: مهمترین ملاحظات دندانپزشکی سالمندان استفاده از روش های با حداقل تهاجم و مواد ترمیمی با نگهداری راحت باتوجه به شرایط اجتماعی، اقتصادی، پزشکی، جنسیت و سن آنهاست. همچنین، لازم است پیشگیری از پوسیدگی و آموزش های لازم به خانواده ها و مراقبان آنها در خانه های سالمندان نیز مورد توجه قرار گیرد.

Factors affecting tooth restoration in geriatric dentistry: A systematic review

Introduction: With the increasing number of elderly people referring to dental clinics, it is essential to have sufficient information for proper treatment. So, the current study aimed to systematically review restorative considerations for the elderly because of limited information in this regard. Methods: In this systematic review, the databases of Pubmed, Embase, Web of Science, Scopus, Proquest, Google Scholar, and Iranian databases were searched with the keywords of elderly dentistry and restorative considerations. Inclusion criteria were original and review articles related to restorative dentistry considerations for the elderly without time limition and in English or Persian. In order to combine the results of the research, thematic content analysis method was used. Results: The total number of retrieved documents was 4208, the number of duplicate documents was 2042, and the number of reviewed documents was 17. Most of these studies were clinical trial, literature review, and retrospective descriptive. Also, most of these studies were conducted in Ireland with four studies, the United States with three studies, and Hong Kong. The Netherlands, Iran, Australia, Finland, Saudi Arabia, Colombia, and Singapore each had one study. In general, restorative considerations in the elderly fall into three categories: considerations related to the elderly themselves, considerations related to restorative methods and materials, and considerations related to dentists' experiences. The most important considerations related to the elderly themselves included attention to medical and social conditions, medication use, type of restorative teeth, level of restoration involved, use of dentures, age, oral cancer screening, education and prevention of tooth decay, and oral hygiene. The most important considerations related to restorative methods and materials were the use of minimally invasive methods such as ART, more durable restorative materials, and attention to the aesthetic and non-aesthetic needs of the elderly in choosing restorative materials. The most important considerations related to dentists were the differences in the use of restorative methods and materials by experienced dentists compared to younger dentists and the rate of referral of elderly people to dental centers. Conclusion: The most important dental considerations of the elderly are the use of minimally invasive methods and restorative materials with easy maintenance and washable according to their social, economic, medical, gender, and age conditions. It is also necessary to consider the prevention of caries and the essential training for families and their caregivers in nursing homes.
